EASTVIEW COMMUNITY FOUNDATION GRANT GUIDELINES

PURPOSE:

The Eastview Community Foundation (EVCF) will award grants that will benefit the Eastview Community in the areas of academics, arts, and athletics based on these guidelined. By targeting different types of applicants and segments within the Eastview Community, these grants will be awarded based on selection criteria that are consistent with EVCF’s autonomous establishment.

ELIGIBILITY:

The following applicants are eligible to apply for grants:

·  Eastview High School attendance area schools:

o  Eastview High School

o  School of Environmental Studies

o  Dakota Ridge

o  Black Hawk Middle School

o  Falcon Ridge Middle School

o  Scott Highland Middle School

o  Diamond Path Elementary School

o  Highland Elementary School

o  Greenleaf Elementary School

o  Oak Ridge Elementary School

o  Thomas Lake Elementary School

·  School-endorsed project representatives, as endorsed by one of the above listed schools.

·  Other organizations proposing to serve as a fiscal agent for a project directed to the support of students (or a student) within the Eastview High School attendance area. There may be an occasion where the indirect recipient of such a grant would be an individual, the determination of which will be at the discretion of the EVCF grants committee.

RESTRICTIONS:

The following applicants/applications are not eligible for an EVCF grant:

·  An organization that is not in good standing with government laws and regulations, including laws and regulations directed to anti-discrimination;

·  A direct grant to an individual (or through an IRS “in-kind” donation);

·  A nationally-directed activity;

·  A debt-reduction effort;

·  A general use by a religious, fraternal, or political organization; and

·  An endowment effort where the funds would later be assessed by another for future possible needs.


CRITERIA:

The grant Application is critical to consideration of any grant. It includes:

·  The Proposal Narrative

·  Program Budget Report

In completing a Grant Application, the following criteria are important:

·  Actual and present financial need that is not likely to be met by other resources, and for a project that is either temporary or without need of ongoing EVCF support.

·  Significant impact of the grant on students of the Eastview community, the Eastview community at large, and the longevity of EVCF.

·  Resource-efficient projects developed in consultation with other organizations to promote coordination and sharing, and to eliminate duplication of services.

·  Matching-funds projects and other efforts defined to encourage widespread community participation.

·  Innovative projects addressing unique or critical needs in the community.

·  A Report Form is due 1 month after projected project completion per the proposal.

4. Grant Narrative Guidelines

Grant Information Required

The following information must be provided in the application narrative:

1. Description of Applicant Organization

Provide a brief history of the applicant organization, a description of the general purpose or mission of the organization, and the scope of its operations, including primary service populations and location of service activities.

2. Statement of Purpose

Explain the general purpose of the project or program for which the grant is requested, the issue(s) it addresses, the significance of the issue(s) and an estimate of the number and geographic locations of those who will benefit.

The Statement of Purpose needs to answer: How does this project or program impact the Eastview Community?

3. Proposed Program Design

Describe (1) the objectives or intended outcomes of the project, i.e., the changes in people or institutions that are expected to occur as a result of the project or program; (2) the activities or set of actions that will be implemented to achieve these objectives or intended outcomes; (3) the information that will be provided in the final report to demonstrate achievement of the outcomes and activities; and (4) the methods that will be used to gather this information. Requests for capital or endowment purposes need to address only (1), (2), and (3).

4. Staff Qualifications

Describe the qualifications and experiences of the principal staff members of the project in relations to its purposes and objectives.

5. Foundation Support

Provide a statement as to why Eastview Community Foundation support is requested; a description of what other efforts have been made (or are presently being made) to obtain the required funds; a list of approved, pending and denied requests for contributions, and the amount received to date for this purpose. Indicate if you anticipate this to be an ongoing/annual request.
