Dear Valued Patient

It has been highlighted in our recent MORI polls that a specific area for improvement is waiting time once inside the practice. We ask that you help us in improving our service by taking a few minutes to fill out this survey and alerting us to problems you have encountered or any comments/suggestions that you may have. The survey has been designed in collaboration with the Patient Representative Group and Patient Participation Group and all entries are completely anonymous. We greatly appreciate your assistance in improving the service we prove.

Yours faithfully,

Mrs Hansa Bakhai

Practice Manager

Q1 When did you last attend the practice?

.

Q2 Who was your appointment with and how long after your scheduled appointment time were you actually seen?

Dr Sawh

Dr Sakthi

Dr Bakhai

Dr Savla

Q3 Have you ever left the practice before being seen because you could not wait any longer? If so, how long did you wait prior to leaving?

Q4 How do you feel about the amount of time you usually have to wait before being seen?

Q5 What do you think would be the best way to inform you of the expected waiting time? Do you have any suggestions for how the waiting time could be reduced?

Most of you wanted a white board in the waiting room specifying individual doctors’ waiting times. This is now operational and is hope is of some help.

Q6 How much time do you think is allocated to a single appointment?

Q7 Did you know that you can book double appointments and, if so, have you ever booked a double appointment? (Seeing the Doctor about two different conditions at the same appointment)

Q8 If a patient misses their appointment slot by arriving late, how do you think the situation should be handled?

Q9 If a patient has a single scheduled appointment and brings in another person (e.g. partner, family member, child), should the Doctor be expected to see both patients in this single appointment given that this may increase the delay for other patients?

Q10 To see how the experiences of our patients vary between different demographics, we ask that you indicate your gender and age group. This information is anonymous and cannot be used to identify you.
