JOB DESCRIPTION

Post:

Independent Living Support Officer

Responsible To: Housing Manager

Purpose:

To provide and a support service to residents in designated housing for older people schemes who have been referred as requiring support .To promote the concepts of “neighbourliness” and “kindliness” within schemes and promote independence .

MAIN RESPONSIBILITIES

  • To take referrals for residents who may require tailored support.
  • To carry out an initial assessment and if tailored support is required to agree with the resident a support plan
  • To carry out on-going assessments and update the support plan periodically until the resident no longer requires the service
  • To be aware of any issues relating to Safeguarding of Vulnerable Adults and report these in accordance with procedures Liaise with other external specialist support providers and refer as appropriate or help the resident access these services
  • Work with residents to tackle isolation, promoting the concepts of ‘neighborliness’ and ‘kindliness’ and linking residents to wider community opportunities
  • Work with residents to enable a range of social activities in the day and in the evening, including the arrangement of outside entertainment where possible
  • Referring residents for any required aids and adaptation and provide any required support whilst aids and adaptations are being installed
  • The delivery of daily or weekly welfare checks to those residents who request this to provide reassurance and support to those who feel vulnerable
  • Providing access to support services that enable residents to remain in their homes
  • Should a resident be unable to continue living independently in their home, provide support during the process of finding alternative suitable accommodation, coordinating with external agencies as appropriate
  • Promote healthy lifestyles including appropriate exercise and diet
  • Promote engagement with activities and hobbies and ensure residents are able to participate in such activities; also promoting the communal facilities linking with external providers who could use them to promote activities
  • Provide temporary help following specific incidents, for example hospitalisation, by providing more frequent visits or ensuring residents have assistance for basic shopping requirements
  • Work with residents to improve their skills, knowledge and confidence so they are able to manage their tenancies
  • Work with residents supporting and promoting independence
  • To collect, record and evidence all information required as part of the contract obligations

Confidentiality

  • The employee shall not either during his/her employment or at any time after its termination:
  • Disclose any information related to any service user or any matter related to their work at the Housing Trust to any person or persons (except to those authorised by the housing Trust to know);
  • Use any information for his/her own purpose or for any purposes other than those of the Housing Trust;
  • Through any failure to exercise all due care and diligence cause any unauthorised disclosure of:
  • Any private, confidential or secret information of the Housing Trust (including in particular lists or details or service user or tenants of the Housing Trust or applicants or relating to the working of any process of the Housing Trust) or which he/she has obtained by virtue of his/her employment.
  • The provisions of the above shall apply mutates mutandis in relation to the private, confidential or secret information of any of the residents of the Housing Trust, which the employee may have received or obtained during his/her employment.
  • All notes, memoranda, records and writing made by the employee relating to the business of the Housing Trust shall remain the property of the Housing Trust and shall be delivered by him/her to the Housing Trust forthwith upon request and/or upon termination of employment.
