2) Is this a Consortium Bid?

The Council will consider bids by consortia. For consortia bids there must be a lead organisation that will be responsible for the overall delivery of the service, and be accountable for the conditions of grant, making payments to partners, accounting for all funds and completing monitoring forms and other information requests. We also require a signed Statement of Intent from all the partners of the consortium, setting out: (a) the lead body (b) policy and procedures of the lead body, (c) the role of each partner in the delivery of the service (d) that the consortium members have agreed to co-operate in making the application, and (e) details of the arrangements the consortium members intend to have for the purpose of the grant, e.g. will it be a partnership, a joint venture or is the intention to form a new company.

14) Service Development Plan (SDP).

All organisations must submit with their application a service development plan covering the funding period (ie 2015 – 2019. It is open to applicants to develop the format for this and it can be set out in tabular form. If the application is from a consortium the service development plan should identify clearly roles and funding for each partner organisation. The SDP must include relevant information regarding the grant criteria on:

·  Outputs, milestones and outcomes for each key activity or service for each grant criteria applied for in each year;

·  If these vary each year, for example due to a reduction in funding, the impact on service users, particularly for disadvantaged equality groups;

·  How outputs and outcomes will be measured;

·  An assessment and identification of risk (high, low and medium) relating to the capacity and sustainability in maintaining the activities and services and how you will mitigate them in delivering the services and activities.

Outputs should be sufficiently detailed regarding the components of the services. For example, if you are running an advice service, you should include outputs for enquiries, advice and casework for each advice category e,g, debt, welfare benefits, housing etc.

If you are applying for more than one grant criteria then, please include all the relevant information on each criteria you are applying for in one Service Development Plan.

Information and guidance on outputs, milestones, outcomes and risk and how to measure these is available from the funding Workers as shown at the start of the application form.
