This document provides additional details for the services we are requesting. This info is in addition to what is posted on the ERate Form 470. Details are on the SLD website.

SAISD is requesting quotes for Internet Access. Please provide costs on a per MB or Gig basis for the following ranges of bandwidth:

·  1 MB – 150 MB

·  151 MB – 300 MB

·  301 MB – 500 MB

·  501 MB – 1 Gig

·  1.01 Gig – 1.5 Gig

·  1.51 Gig – 2 Gig

SAISD is currently subscribing to 150 MB of internet bandwidth. SAISD anticipates increasing internet bandwidth incrementally (including possibly increasing at levels within the ranges listed above).

It is possible that certain parts or pieces of a system or service may be ineligible for purchase under the Erate guidelines. Please note on your quote any ineligible portions of your quote.

SAISD will consider contracts that are a multi-year contract and/or a contract featuring voluntary extensions in 1 year increments up to 3 years.

Please refer to Eligible Services List at: http://www.universalservice.org/sl/ for a detailed explanation of what is eligible and what is not. The request is only for eligible services or portions of eligible services.

Please submit contracts with the submission of your quote. Contracts must be in electronic format and editable in Microsoft Word.

Bid Evaluation

·  See items listed in #14 of the “General Conditions for Bidding and Required Forms” document

·  In addition, the following criteria may also be used:

o  Support

o  Conformity to quote specifications

o  Prior experience implementing funded projects via the ERate grant program

o  Integration with existing network infrastructure

FYI: a multi-year contract was signed in a prior E-Rate year for the following items: Local Telephone Service, Long Distance Service, Wide Area Network (Digital Transmission Service), Email Services, and for Cable Modem service for some of the SAISD sites.
