Immediate Action Required

A key part of the Leadership Nomination Program you will be attending is the opportunity for you to receive feedback about your leadership behaviors based on the ExxonMobil Leadership Framework. This feedback will be gathered using the ExxonMobil Leadership Practices Questionnaire (LPQ).The purpose of this process is to help you increase your leadership effectiveness through feedback from your manager(s), peer interfaces, and direct reports / team members that you select. The LPQ takes about 15 to 20 minutes to complete.

The LPQ information collected will be confidential. To ensure confidentiality, only aggregate data will be provided to you from your peer interfaces and direct reports / team members. You will receive individual feedback from each manager you select.

You are the only ExxonMobil person who will receive your individual results. You will receive your individual results during the program session for use in preparing a Personal Leadership Development Plan. Your Program Faculty will work with you during the program to help you (a) analyze your LPQ results and (b) provide assistance with your personal leadership development planning. No post-program or on-the-job coaching is included as part of the leadership program.

Vector Data Services (VDS), an external firm, will coordinate the data collection process. Input will be collected on a confidential basis using their secure technology.

In order to initiate the LPQ process, please follow these actions:

  1. Read theData Privacy Statement for Personal Informationandcomplete the sentence at the end of this Data Privacy Statement to indicate your agreement to its provisions (Page 2)
  1. Complete the LPQ Respondent Information Form(Page 3)

  1. Once VDS receives your completed Forms, they will contact yourselected respondents via e-mail and provide instructions for completing the LPQ for you
  1. You will need to notify all of your selected respondents that you have selected them to provide feedback on your observed leadership behaviors and that they will receive an e-mail from VDS with instructions on how to provide LPQ feedback for you

Important note

The LPQ is an integral part of this Leadership Training Program. Receipt of sufficient LPQ feedback data (including your own self-assessment) is required for program participation

Ifthis LPQ submission deadline is not met,the full tuition for this program will be charged to your business as a cancellation feeand you will need to o see if it is possible to reschedule you to a later session

Data Privacy Statement for Personal Information

The Leadership Practices Questionnaire ("LPQ") is a key component of ExxonMobil Leadership Training programs. The LPQ elicits information on the participant's leadership behaviors (see Attachment I) from each participant and from participant-identified raters. Your agreement to the following terms is needed below.

LPQ information will be submitted electronically over the internet through access to a secure (password protected) web site managed by Vector Data Services, Inc. (VDS), a U.S. company and ExxonMobil contractor. A written form that is faxed to VDS will be used by those who do not have internet access.

LPQ data will be used to generate (a) individualized feedback reports containing information about you for personal leadership development planning and (b) summary reports--with no personally identified information--for program faculty and/or ExxonMobil planning of leadership development initiatives. Individualized reports and information are confidential and will only be provided to you, the participant.

By responding to the LPQ, you will be giving your permission to use information and data about you submitted by you and your LPQ raters as set out in this document, including receipt and use of your personal information by your coach(es), and you agree to the following statement:

I freely and voluntarily consent to the transfer, at any time, of LPQ information and personal data about me to ExxonMobil's data collection and processing organization or contractor and to leadership training program faculty and coaches, wherever located, regardless of whether the country in which the organization, contractor, faculty member or coach is located has an adequate level of protection within the meaning of Directive 95/46 of the European Union or any similar national directive or law.

The security of personal LPQ information is important. Consequently, reasonable precautions have been taken. Vector Data Services, Inc. limits access to LPQ information to its database administrators and its subcontractors. All LPQ information, including information you and your raters provide, is collected, processed and stored in VDS-controlled databases located in the United States. VDS is obligated by contract to ExxonMobil to ensure an adequate level of protection for rights and freedoms of participants regarding processing of personal data, to use personal LPQ information only for leadership training purposes and not to disclose such information to any person(s) other than the participant to which it relates, unless required to do so by a valid court order, governmental agency order or information request, or unless there is a legitimate business purpose for doing so and the third party agrees to similar restrictions on security, disclosure and use of the information. VDS discloses personal LPQ information to its contractors in the U.S. that store and process information for purposes related to leadership training programs, and those contractors are obligated in writing to restrictions on security, disclosure and use of the information at least as stringent as VDS' obligations above.
