JOB DESCRIPTION

Job Title: Practice Nurse

Accountable to: Partners (Clinical) / Practice Manager (Administration)

Hours: 16/30 hours per week

Job Summary

The purpose of the role is to:

  • To compliment the role of the GPs and other health professionals as an integral and key member of the care team
  • To make the best use of clinical skills within the practice in order to promote wellbeing and maximise health gain for patients
  • To provide a high quality and accessible nursing service to the practice population

Job Responsibilities

Professional:

  • Provide comprehensive annual reviews to patients with chronic diseases
  • Take a pro-active approach to utilising the dietitian and podiatrist clinics for diabetic patients
  • Assess problems presented opportunistically by patients
  • Provide continuing care to patients across the practice population
  • Initiate laboratory requests and collect specimens as required, interpret results and act accordingly
  • Performa holistic assessment of patients attending for ear irrigation
  • Provide a holistic approach to wound management and implement wound care in line with current evidence based guidelines, and locally agreed formulary
  • Suture removal
  • Perform a holistic assessment of patients attending for cervical cytology smear tests
  • Advise about vaccinations and injections and ensure they are administered under relevant policies and guidelines
  • Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including vaccinations, medicines, safe sex, food hygiene, sun protection, etc.
  • Assist in the provision of minor surgery
  • Recognise and manage anaphylaxis according to current UK guidelines
  • Be able to perform cardio-pulmonary resuscitation according to current UK guidelines(updates regularly provided)
  • Obtain and document informed consent (either verbal or written)
  • Ensure infection control guidelines are maintained
  • Communicate effectively with other members of the Primary Care team
  • Preserve confidentiality of information gained at work at all times
  • Any other duties considered relevant to the post

Managerial:

  • Contribute to the assessment of service needs
  • Assist patients to identify their health needs
  • Maintain accurate records in line with practice policy and NMC guidelines using appropriate computer software
  • Responsible for maintenance of equipment and stock control includingrefrigeration, vaccine ordering, single use equipment, general treatment room equipment and stock, andemergency equipmentand drugs
  • Take a pro-active approach in supporting the recall of patients for annual chronic disease reviews
  • Contribute a nursing perspective to the Practice development plan
  • Manage and organise individual patient consultations
  • Attend staff meetings
  • Effectively manage own time, workload and resources
  • Preserve confidentiality of information gained at work at all times

Educational:

  • Identify personal development and training needs in conjunction with the GP/Manager
  • Participate in an annual individual performance review, including taking responsibilityfor maintaining a record of own personal and/or professional development
  • Maintain NMC registration and continuously work towards achieving revalidation
  • Participate fully in practice audit and quality development programmes
  • Provide mentoring support to the Healthcare Support Worker
  • Participate in formal tutorials and informal training of student nurses and GP Registrars in the practice

Health & Safety:

The post-holder will assist in promoting and maintaining their own and others’ health, safetyand security to include:

  • Identifying the risks involved in work activities and undertaking such activities in a waythat manages those risks
  • Making effective use of training to update knowledge and skills
  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
  • Reporting any potential risks that are identified
