North West Dementia Network Excellence in Dementia Award

This award is intended to recognize outstanding commitment of individuals in improving the lives of people living with dementia. Each year, two individuals will be awarded the Excellence in Dementia award—one from within the city of Thunder Bay, and one from the region (i.e., across the NWLHIN).

The recipient will be someone who demonstrates creativity, innovation, dedication, and excellence in improving the lives of people living with dementia, whether through enhancing health and/or social care, education activities, research, advocacy initiatives, or volunteering. People living with dementia, care partners, health and social service providers, educators, researchers, and volunteers are all eligible for the award.

The following criteria may be used:

  • Someone who has advocated and worked to change perceptions and stigma of dementia
  • Someone who has championed innovative health or social care practice in supporting people living with dementia
  • Someone who goes above and beyond in caring for and supporting people living with dementia
  • Someone who has made a significant impact in the lives of people living with dementia
  • Someone who is a positive role model for others in dementia care
  • Someone who has advanced practice in enhancing the quality of care and services for people living with dementia
  • Someone who has influenced change and is a strong leader in dementia care

Individuals must be nominated for this award. Upon permission from the person you are nominating, the nominator must submit the following:

  • the nomination form (attached);
  • Summary of Contributions—amaximum of one page description of why the individual should receive the award must be provided to the selection committee. The description should include the nominee’s achievements in detail, including directly outlining how the nominee has contributed to excellence in improving the lives of people living with dementia. Specific examples will be helpful.

Letters of Reference—A maximum of two letters of reference may be included in the nomination package. Letters should detail the nominee’s contributions and achievements, directly outlining how the nominee has contributed to excellence in improving the lives of people living with dementia.
