6. BRIEF RESUME OF INTENDED WORK:
6.1NEED FOR THE STUDY:-
The man is precious creature on the earth he want to go for diversifiedto enjoy the life to simply lie idle, for such activities harmony of musculoskeletal system is the minimum requirement.Now aday thisdisease has become a major health problem in 40 years and above age. Based on the signs and symptoms, sandhigata vata may be correlated with osteoarthritis of modern science.
According to World Health Organization (W.H.O.) osteoarthritis is the second commonest musculoskeletal problem in the world population (30%).The reported prevalence of OA from a study in rural india in 5.75%1. Hence it is a challenge to each system of medicine to provide the effective treatment to control it. Ayurvedic line of treatment in Sandhigata vata is swedan ,upanaha, mardan and snehan2.Sweda karmaposseses stambhaghna, gauravaghna and sheetaghna qualities3.which seems to be effective procedure for osteoarthritis. Both patra lavana pinda sweda and swedopag mahakashaya pinda sweda have shothahara and vednasthapan qualities. So the present study is planed to assess the efficacy of Sweda karma by a comparative study on patra lavana pinda swedaand swedopag mahakashaya pinda sweda for the management of sandhigata vata w.s.r. to osteoarthritis.
6.2REVIEW OF LITERATURE:
1)General nidana of vata vyadhi are deemed to be the etiological factorsof sandhigatavata as it is delt under nanatmaj vata vyadhi4.
2) Due to vatakara ahara vihara vata get vitiated and lodges in rikta srotas i.e srotas where abhava of snehadi guna is present. Due to vitiated vata shleshmaka kapha in the joint also decreases in quality and quantity i.e ruksha laghu kharadi gunas will over power producing disease sandhigata vata.
3)According to charaka if vayu is located in joint, there occurs swelling like air – filled bladder in touch and pain during constraction and extension 5.
4)According to sushruta,vata localised in sandhi (bony joint) it destroys the action of the joint and give rise to pain and swelling in them6.
5)Swedana , upanaha , mardana and snehana are the choice of treatment sandhigatavata2 .
6)Patra lavan is explained in vata vyadhi chikitsa by sushruta7.
7)Swedoupag mahakashaya drugs are explained in sutra sthanashadhvirechaniya satashritya adhaya by charaka8.

6.3Objectives of the STUDY:-
- To assess the efficacy of Patra lavana pinda sweda in sandhigata vata .
- To assess the efficacy of swedopaga mahakashaya pinda sweda in sandhigatavata.
- Tocompare the efficacy of Patra lavana pinda sweda and Swedopaga mahakasaya pinda sweda in the management of sandhigata vata .
MATERIALS AND METHODS:
7.1SOURCE OF DATA -
1.) Patients will be selected from Sri Siddharoodha Charitable Hospital,
attached to N.K.J.Ayurvedic Medical College & P.G. research Centre Bidar, Karnataka .
2.)Ayurvedic Classics andmodern books, Journals, magazines, national and internationalseminars, conferences, digital library & web sites.
3) Materials – drugs will be collected from the Herbal garden and Ayurvedic pharmacy under the supervision of Dravyaguna specialist.
4) Rasashala attached to the college for preparation of medicine under the supervision of Rasashastra expert. .
5) The Sweda karma will be conducted under the supervision of our guide and co- guide in Panchakarma Theater.
7.2METHODS OF COLLECTION OF DATA:
This is a comparative clinical study in which 30 patients (including both group) will be selected on the basis of simple randomized sampling procedure according to inclusive criteria.
RESEARCH DESIGN:
Total 30 patients will be selected in study15 patients will be distributed in two equal groups such as
- Group- A:- 15 Patients will be treated with Patra Lavana Pinda Sweda for seven days.
- Group-B: - 15 patients will be treated with Swedopaga Manhakashaya Pinda Sweda for seven days.
Separate case sheets will be designed for this purposeBefore starting treatment through counseling of patient and brief explanation regarding the procedure of swedana karma will be done.
In both the groups patients will be advised to take the laghu & anabhisyandi ahara and advised to avoid exercise during the entire course of swedana karma.

- In both the groups patients will be advised to remain on light diet (anabhisyandakarahar) and advise to avoid exercise during and after the entire course of sweda karma.
Follow up -
Two follow ups will be conducted in the 7 days interval i.e. 7th and 14th days after the comption of treatment schedule .
METHODOLODY:-
The progress will be noted on the basis of assessment parameters (both subjective and objective) before treatment & after treatment in a specially prepared case sheet.
STATISTICAL ANALYSIS:
The observation will be analyzed critically & scientifically by employing some statistical tests between the groups and also within the groupwith suitable statistical analysis.
SELECTION CRITERIA:-
INCLUSION CRITERIA:-
1. Patients of more than 40 years of age irrespective of sex.
2. Patients with sign and symptoms of sandhigata vata.
3. Patients who are eligible for swedana karma.
EXCLUSION CRITERIA:-
- Patient with other joints pathologies like RA, T.B bone etc. and suffering from systemic acute pathogenesis
- Non co-operative patient.
3.All the condition in which Sweda karma is contraindicated.
PARAMETERS FOR ASSESSMENT:-
SUBJECTIVE PARAMETERS:-
1) Sandhi Vedana (pain)
2) Sparsha ashahishnuta (tenderness)
3) Atopa (crepitus)
4) Prasaranakunchan pravrati sa vedana. (pain on movement)
5)Sandhi sopha (swelling like vata purn druti)
OBJECTIVE PARAMETERS:
1) Range of Motion on the basis of Goniometer reading.
ASSESMENT CRITERIA:
Improvement in the sign and symptoms of the patient mentioned in subjective and objective parameters with due importance to Samyak Swinna Lakshana.
INVESTIGATIONS REQUIRED:
- X-ray ofknee joint AP/PA view.
- CBP, ESR .
- MRI if necessary.
