THIS SURGERY IS A GP TRAINING PRACTICE

You may be offered an appointment with one of our GP trainees. These are fully qualified doctors who are undergoing training and supervision to become a GP. If you are not happy seeing one of these doctors you can ask to see one of the usual doctors.

TO REGISTER AT THE SURGERY

Please complete one of our registration forms and hand it to a receptionist. These can be collected from reception or printed from our web site. Please make sure that you live within our practice area (see map).

As a patient of this practice you are able to see any of the doctors or other health care professionals as long as an appointment is available, however we suggest that you try and see the same doctor consistently and any preference expressed will be noted in your medical record. On occasions, especially in an emergency, you may not be able to see the doctor of your choice.

We operate a booked appointments system. Any registered patient may ring 0208674-6376 to make an appointment. If you are late for an appointment we may have to ask you to re-book. If you need to cancel your appointment please give us as much notice as possible so that we can offer it to others.

Home visits:

These should only be requested by patients who are too ill to travel to the surgery. Please phone by10.30 a.m. (The earlier you phone the easier it is for us to organise services). Pleasecome tothe surgery if you can, the doctor can see 4 people in surgery in the time it takes to see one at home.

Emergency consultations:

If you are suddenly taken ill and cannot wait until the next available appointment, please phone at 8.15 am to arrange a time to be seen during morning surgery for treatment of the urgent condition. For long term/non-urgent illness please see your own doctor in a routine appointment

WHAT TO DO WHEN THE SURGERY IS CLOSED;

If you need urgent care or advice outside surgery hours and at weekends there are several options.

You can visit a local pharmacist (chemist) and ask for advice.

If you need to speak to a doctor urgently you can ring SELDOC on 0208693 9066.

Saturday between 7am – 11 am phone on 0845 602 4464

If you need emergency treatmentin a critical or life threatening situation you should go to the nearest A&E Department or if necessary ring 999 for an ambulance.

TEST RESULTS

Please ring between 3.00 p.m. and 6.00 p.m. for the results of tests.

REPEAT PRESCRIPTIONS

To request a repeat prescription please send or hand in your computer prescription request or a letter informing us what you require, giving at least 2 working days notice. You can also fax your request on our prescription onlyfax line 020 8671 0283, this also requires 2 working days notice. We are unable to take repeat prescription requests over the phone.

SPECIAL SERVICES AND CLINICS

The practice nurses are available for health checks, blood pressure check, dressings, ear syringing, cervical smears and stitch removal. They will give advice to all patients on stopping smoking, cutting down drinking and staying (getting) fit. They also see patients with asthma and high blood pressure. Blood tests are currently only available to the elderly or infirm. Please make an appointment in the usual way.

MINOR SURGERY

Some minor surgery is undertaken within the practice.

CONTRACEPTION

All the doctors can provide contraceptive advice and services. They can discuss the best method for you, including the fitting of coils and diaphragms.

Post coital contraception (emergency) is also available within 72 hours.

All contraceptive advice is confidential for all our patients including teenagers.

CERVICAL SMEARS

These should be done every three years. Please book an appointment with a practice nurse.

TRAVEL VACCINATIONS

We provide a travel vaccination service. Please ask at reception for information on how to obtain travel advice and services.Travel appointments should be made at least 6-8 weeks prior to departure.

Non NHS Services:

Certain services are not provided under the NHS such as private referrals, some vaccinations, private certificates and prescriptions. For information on charges please ask at reception.

ANTE NATAL CLINIC

Tuesday 9.45a.m. - 13.30 p.m. with the midwife.

CHILD HEALTH CLINICS

Thursday: 1.00 p.m. - 3.00 p.m.

6 week check and immunisation appointments are arranged at reception. No appointment is necessary to see the Health Visitor or for weighing but appointments for developmental checks are arranged via a Health Visitoron 02030494003

Please do not bring sick children to these clinics, but to routine surgery.

COUNSELLORS

A counselling service is available at the surgery. Patients can be seen only by referral from their GP.

CONFIDENTIALITY AND DISCLOSURE OF RECORDS

You have a right to privacy and confidentiality and everyone working in the NHS has a legal duty to respect this. We only ever share information about you to others who have a legitimate need for it, in accordance with the Data Protection Act. They also have a legal obligation to keep it confidential. We send patient information, such as childhood immunisations, smears and mammograms to the Primary care Trust screening service as this information is held on a central NHS database.

We will not disclose any personal information to any person who is not involved in your care without first obtaining your written consent.

NAMED GP

All patients are allocated a named GP when they register. If you cannot remember which GP you were allocated, please ask one of the receptionists. The allocation of a named GP doesNOT mean that you can only see this doctor, you have a choice to see any GP.

COMMENTS

We welcome comments and suggestions on the services we provide. If there is anything you particularly wish to raise, please let either your doctor or the practice manager know. Should you feel the need to complain about our service you can obtain a leaflet on what to do, at reception.
