WELCOME TO THE PRACTICE

In the practice we work as a team with the reception and administration staff, practice and community nurses, health visitors and midwives.

We have surgeries at The Avenue in Alwoodley, Leeds 17 and Green Road, Meanwood, Leeds 6. Your paper records are kept at one surgery but your computer records can be accessed from either surgery. For administrative reasons we prefer you to use the surgery at which you registered. If you wish to transfer your records to the other site please let us know.

There is disabled access at both surgeries.

The practice does not undertake the teaching or training of health care professionals or persons intending to become health care professionals.

PRACTICE BOUNDARIES

A map is available in the surgery and on the website which shows the Practice boundaries. Most of Leeds 17 together with parts of Leeds 7, Leeds 6 and Leeds 16 are included.

MAKING AN APPOINTMENT

We operate an appointment system for allsurgeries; some of our doctors' appointments are available to book up to 4 weeks in advance. Nurses and Health Care Assistant appointments can be booked further in advance if required.

Werelease approximately 15% of doctors’ appointments for advance booking at the beginning of each month; a further 15% of appointments are released on a Monday morning for the following week. The remainder ofthe appointments are released each morning and can be booked for the sameday; we always ensure that anyone needing to see a doctor urgently will be seen, or assessed bytelephone the same day.

An appointment can be made by telephoning the surgery, coming to the reception desk or booking on line.

Please note that all appointments are for one person only.Some of the appointments are to discuss one problem only to help the doctor from running late when patients need to discuss more than one medical issue.

You are free to choose which doctor you see; just ask when making the appointment. However, if a doctor has started treating you it is better to continue seeing that doctor until your treatment has finished.

TELEPHONE ADVICE

You may phone the surgery during working hours and ask to speak to a doctor or a nurse. You may be asked to ring back later as the doctors and nurses cannot take telephone calls when they are seeing a patient. Receptionists may be able to deal with some queries or advise on the best time to speak to a doctor. Alternatively, you may send a request for a telephone call via our home page on this website.

NHS Direct is available 24 hours a day for health information and advice from a nurse on telephone number 111 and on-line at .

REGISTERING AS A PATIENT

Patients may apply to join the practice list by completing a registration form and health questionnaire, available from reception. Patients will be informed, in writing, within 10 working days if we are unable to accept them.

ACCESSIBLE INFORMATION

If you have a disability, impairment or sensory loss you can request to receive information in a format that you can access and understand, for example in large print, braille or via email, and also professional communication support if you need it, for example from a British Sign Language interpreter.

HEALTH CARE ASSISTANT

Many Krushell – is a trained phlebotomist (blood taking) and can also undertake other duties to support the nurses.

DISTRICT NURSES

The district nurses provide a wide range of nursing care.

They are based at Meanwood Health Centre – Tel: 0113 8433350 (between 8.30am and 5.00pm including weekend and bank holidays). Out of hours number is 0300 003 0045.

HEALTH VISITORS

Health Visitors are based at The Reginald Centre and can give advice on health care for mothers, babies and children up to school age.

MIDWIFE

The midwife runs our ante-natal clinics at The Avenue Surgery and gives advice on all aspects of health for mothers-to-be and on the care and feeding of newborn babies. She can be contacted via our reception staff or during ante-natal clinics (Thursdays weekly).

HOME VISITS & EMERGENCIES

Home visits are only undertaken in exceptional circumstances. To request a home visit please telephone 2953790 – for patients at both surgeries. Please try to phone before 10.00am.

For emergencies, at any time, please phone 2953780. You may hear a recorded message telling you to ring another number. Please have a pen and paper handy and listen to the message to the end.

WHEN THE SURGERY IS CLOSED

If you call the surgery when we are closed, and need to see or speak to a doctor urgently, you will be given an out of hours number to call. Alternatively, call NHS Directorsee NHS Choices for further information about local walk incentres and/ or Minor injuries Units

Call 999 in an emergency. Chest pains and / or shortness of breath constitute an emergency.

REPEAT PRESCRIPTIONS

The surgery may give you a repeat prescription list if you are getting regular prescriptions. To order your repeat prescriptions pleaseorder via our website, or bring or send your list (enclosing a stamped addressed envelope if you wish us to post your prescription either to yourself or a pharmacy) to the surgery. Usually your prescription will be ready 48 hours later. Prescriptions can be ordered and collected from reception during our opening hours. In order to avoid mistakes, we do not accept requests for repeat prescriptions by telephone. Most treatments need to be monitored so sometimes when you ask for a repeat prescription you will be asked to make an appointment with a doctor or a nurse.

LABORATORY SPECIMENS

Laboratory specimens must be handed in at the reception desk before 4.00pm Monday to Friday. Please make sure that your name and date of birth are written on the bottle and the form because the laboratories will not process unnamed specimens. Please note that patients are entitled to complete confidentiality; we are unable to divulge any information from patients’ records or laboratory results to any third party without the express written consent of any patient aged over 16 years. ANTE-NATAL CLINIC

Clinics are held by a midwife at The Avenue. Please make an appointment.

BABY CLINIC

The Avenue Surgery – 2nd and 4thWednesday of each month from 10.30am to 11.15am

The baby clinics are held by a doctor, nurse and clerical assistant for child developmental checks, immunisations and weighing. Please do not bring children to Baby Clinic if they are ill.

ASTHMA & DIABETES CLINIC

Clinics are held by the practice nurses at both surgeries – please make an appointment. Please note – if you are attending the Asthma Clinic you should bring your inhalers with you.

FAMILY PLANNING ADVICE

We are happy to discuss all aspects of family planning and carry out cervical smears etc. Young people can be absolutely sure that anything they discuss with members of the practice team will stay confidential and nothing will be said to their parents, teachers etc without their permission. Contraceptive Implants are available – appointments to be booked with Dr Hardy.

SMOKING CESSATION ADVICE

Over 100,000 people die in the UK annually from smoking related illness. Advice and support, and prescriptions if necessary, are available. Please telephone reception for further information on local services.

TRAVEL IMMUNISATION AND VACCINATIONS

The practice nurses offer a wide range of vaccinations and travel advice. Please make an appointment, preferably eight weeks in advance of your holiday, to ensure adequate protection.

FLU AND PNEUMONIA IMMUNISATION

An influenza vaccination is particularly recommended for patients aged over 65 and those younger patients who have heart, lung or kidney disease, diabetes and also residents of nursing and rest homes and Carers. Eligible patients will be contacted in October annually with details of when the clinics will be held.

Patients aged 16 – 74 years who have not seen a GP in the last three years are entitled to a health check with the practice nurse.

Patients aged over 75 who have not seen a GP in the last year are entitled to a health check with the practice nurse.

MINOR INJURIES

NHS Leeds has chosen not to commission a Minor Injury service from GPs. Therefore, any patient with a minor injury must attend a hospital A&E Department or a Minor Injuries Unit for further care and advice.

PRIVATE SERVICES

Some services are not available on the NHS. Private medicals (for employment, elderly drivers, sports and some types of insurances) must be paid for by the patient. There are notices at reception to inform patients of the current charges. If you are unsure – please ask!

WHEN SHOULD YOUR CHILD BE IMMUNISED?

Children’s’ immunisations are performed by the Nurses during the Baby Clinics – no appointment necessary. Please visit for more information.

OVERSEAS TRAVEL

Travelling abroad is so common these days that it is often easy to forget the necessary precautions. Consult our practice nurses at least two months before departure. They will advise you what vaccinations and medicines you may need. Make sure that you have adequate health insurance. If you are visiting an EEC country you will need a European Health Insurance Card (EHIC). Application forms are available at Post Offices or apply by telephone to the NHSBSA EHIC Line on 0845 606 2030 or you can complete your application ‘on-line’ at . Take a small first-aid kit with you and beware of bites from insects and animals as many diseases are insect-borne and rabies is widespread. Be careful with raw vegetables, salads, fruit, shellfish and ice-cubes. Respect the power of the sun! Use suncreams regularly (at least Factor 25 for children) and wear a hat.

Sexually transmitted diseases (including AIDS) are a major health threat throughout the world. Condoms offer some protection. If your Doctor advises you to take anti-malarial tablets remember that you need to start taking them one or two weeks before departure and continue until four weeks after your return. These tablets are available from a Pharmacy and are essential if you are traveling to areas where malaria is present. Please note that any medications prescribed for holiday use are only available on private prescription

PATIENT INFORMATION

Only essential and relevant information will be shared with other health care agencies. Checks will be made to ensure that the person requesting the information is legitimately entitled to request the information. Policies are in place to ensure that staff are aware of the risks of breaking confidentiality and know how to minimise these risks. Information will not be given to any person or agency that is not concerned with health care unless express consent is obtained from the patient concerned or a responsible adult in the case of a child.

COMPLAINTS PROCEDURE

It is this practice’s policy to deal with complaints promptly, fairly and, wherever possible, at practice level. In case of complaint you should contact Lynne Doyle, Practice Manager, either in person or in writing. She will consult with other practice members to ensure that your complaint is dealt with in an appropriate manner. This may involve your being invited to a meeting at the practice.

DEALING WITH VIOLENT OR ABUSIVE PATIENTS

We expect all people to behave politely and courteously towards our staff, and other people on the practice premises, at all times. When this does not happen, we shall take action against the individuals concerned. This will involve removal from the premises and/or prosecution. Such patients will be removed from the practice list.

PATIENTS’ CHARTER

We aim to provide our patients will the best quality care available. Our Charter is a statement of what you can expect from the practice and what we feel we can expect from you in return.

  1. You can expect to be greeted in a polite and welcoming manner, at all times, by the Doctors, Nurses and other practice staff.
  2. You can expect the surgery to be clean and comfortable as far as facilities allow.
  3. New patients will be offered a health check when they register with the practice.
  4. You have the right to be referred to a consultant, acceptable to you, when the Doctor thinks this is necessary.
  5. You have a right to confidentiality at all times
  6. The Doctors and Nurses aim to see patients within 30 minutes of their appointment time but where there is a delay you can expect to be told the reason by a receptionist.
  7. You have a right to a copy of the practice leaflet, which contains information about the services we provide.
  8. You have the right to see your medical records subject to the limitations of law.
  9. The Practice Manager will deal with complaints in the first instance

With These Rights Come Responsibilities

  1. To treat all practice staff with the same courtesy you expect from them
  2. To ensure that you attend for you appointment on time and, if you need to cancel, please give adequate notice so that someone else may have your appointment.
  3. Requests for advice or help on non-urgent matters should be made during surgery hours.
  4. Home visits should only be requested by or on behalf of seriously ill patients.
  5. Requests for home visits should be made, whenever possible, before 10.00am by phoning 0113 2953790.
  6. Requests for night visits should only be made when they are absolutely necessary.
  7. You should work with the practice staff to keep yourself as healthy as possible by following advice from the Doctors and practice staff.
  8. You should not expect a prescription at every consultation.
