Completed applications should be returned to myself by the close date of Monday 24 August 2015. We anticipate holding interviews on Tuesday 1 September 2015.

Due to the large number of applicants that we are likely to receive, I am afraid that we will not be able to respond to everyone; therefore, unless you are called for interview you will not receive any other acknowledgment.

As the role will involve both opening the Surgery on some days at 7.45am and closing up some days at 6.00pm, or occasionally 7.15pm, this post will only be suitable to those who can attend at these times. If you rely on public transport, please ensure that you would be able to get to work to meet these times before applying.

Instructions for completion of application form

Please read this carefully before completing your application form.

1.Application Form

  • Only completed Southside Road Surgery application forms are eligible for short listing.
  • Complete all sections of the form.
  • Do not send your CV.
  • Sign your application form and return it either by post or deliver it in person.

2.Closing Date

  • To be considered, you must return your application on or before the closing date.
  • Applications received after the closing date will not be eligible for short listing.
  • Please make sure you pay the correct postage. Southside Road Surgery will not pay underpaid mail.

3.References

  • Please include your current or most recent employer. If you feel you cannot ask your current/most recent employer, we will discuss this with you.
  • Friends or relatives are not acceptable as referees.
  • References are only taken up if you are offered a post with us.

4.Statement In Support Of Application

  • Tell us something about yourself, your skills and achievements.
  • Try to think about why you want this post and what skills/attributes you have that would make you suitable.
  • This section is used to help us decide whether to offer you an interview or not so please make sure you complete it as fully as you can.

5.Short Listing and Interviews

  • If you are short listed for interview, we will usually contact you by ‘phone. Please make sure we have the correct details for you.
  • We have a limited number of interview times. It will not be possible for us to offer an alternative interview day. If you cannot attend at the time offered, we may be unable to offer an alternative. In this case, you may wish to withdraw your application.
