PARTICIPATION FORM
TO SUBMIT A WINE ENTRY:
· Please complete this participation form in its entirety.
· Immediately send a copy by fax to the Competition organizer ASPERIA at +39. 0131 313250.
· Put the wine samples into a case which is suitable for overseas delivery and insert the following into the case:
o A copy of the participation form.
o A copy of a valid Identity Document of the legal representative.
o Six (6) labels for each wine submitted to the Competition.
· Affix an obvious label on the box which says “4th BARBERA COMPETITION”
· Deliver it to the collection center in your country (reception deadline September 20th,2008) or directly to Italy (reception deadline October 15th, 2008) – see details at the end of this form.

DECLARATION
BEING AWARE of the punitive measures imposed by Italian law concerning false declarations or the use of false data,
I DECLARE:
§ That the wine has the characteristics detailed above.
§ That the samples for each entry are all sourced from the same lot.
§ That the samples for each entry have been taken by the declared company representative (6 bottles for each entry) on behalf of Asperia, organizer of the Fourth International Barbera Competition.
§ That I have read the Regulations of the FOURTH INTERNATIONAL BARBERA COMPETITION and accept them in their entirety.
If an inspection is required to confirm the validity of the statements declared above, the producing company will grant personnel acting on behalf of Asperia access to the winery building for control procedures.
If there is any discrepancy between information declared above and information later verified during control procedures, the winery may face legal consequences and will be excluded from the Competition.
