  1. UNDERSTANDING SERVICE CONTEXTS

To influence leadership environments, it is essential to understand the service context. This is difficult to achieve because we are part of that context so it is vital to develop conscious competence through the application of psychology and generalising clinical skills. It is important to have an understanding of the organisation, policy and professional contexts as well as the process of service transformation as change and improvement are a continuous part of the landscape. Both critical reflective and values based practice, drawing upon the evidence base, guide this process of continuously seeking to understand and make sense of service context.

1)UNDERSTANDING SERVICE CONTEXTS - Learning outcomes

Working in Systems series

Explain the importance of understanding organisational systems, particularly with reference to the core professional and organisational competencies required of clinical psychologists (GMC PF OSIL)

Trainees will be able to:

Demonstrate an awareness of the legislative and national planning context of service delivery and its impact on clinical practice and the meaning of different policy initiatives. (GMC, OSIL)

Understand the impact of governmental policy and other key influences e.g. The NHS Constitution; National Institute for Health and Care Excellence; The Increasing Access to Psychological Therapies (IAPT) programme; The Health and Care Professions Council; The British Psychological Society and its Division of Clinical Psychology. Trainees will be able to:

Understand the NHS structures, organisations and purposes. (GMC, OSIL)

Service transformation

Trainees will be able to:

Understand the legislative and national planning contexts for service delivery and clinical practice in which service transformation can occur (OSIL)

Develop the capacity to adapt practice to different organisational contexts for service delivery, as service transformation requires a whole system understanding. (OSIL)

Develop the capacity for indirect influence of service delivery to facilitate service transformationby bringing psychological influence to bear in the service delivery of others (OSIL)

Become effective at working with users and carers to facilitate their involvement in service planning and delivery as part of service transformation (OSIL)

Understand the of change processes in service delivery systems (OSIL)

Understand and work with quality assurance principles and processes including informatics systems that may determine the relevance of clinical psychology work within healthcare systems. (OSIL, E)

Recognise malpractice or unethical practice in systems and organisations and know how to respond to this, and be familiar with ‘whistleblowing’ policies and issues. In addition to recognise the importance of positive approaches (for example appreciative inquiry) for facilitating service change (OSIL)

3)INFLUENCING SERVICE CONTEXTS

The ROP strand defines leadership as influencing the service context; leadership is conceptualised as dynamic, existing in the interaction of people in context, and equally important to clinical psychologists in designated (formal) and non-designated (informal) leadership positions. There are a number of roles frequently taken on by clinical psychologists, such as consultation, which can be understood as leadership opportunities, so learning to be influential in these roles through the application of psychology and generalising clinical skills is important. Trainees need to develop their own unique skills of influencing service context; this process is supported by an examination of models in the literature, models in practice and the competencies required by the profession. Both critical reflective and values based practice, drawing upon the evidence, guide these processes.

Trainees will be able to:

Explain the importance of consultation in clinical practice, particularly with reference to the core professional and organisational competencies required of clinical psychologists (GMC OSIL)

Explain the importance of negotiation, preparation and review with consultees in both formal and informal settings so a shared efficient process is achieved (GMC CT E OSIL)

Distinguish between process and content expertise in the consultancy role (GMC PF)

Explain how consultation can be undertaken within a framework of the core therapeutic models (GMC PA PF PI)

Understand social approaches to intervention; for example, those informed by community, critical, and social constructionist perspectives (GMC I PPSV OSIL )

Locate leadership approaches in the leadership models literature and therefore recognise there is no one model of leadership but a dominant model which will change over time (GMC, OSIL)

Trainees will be able to:

Develop an appreciation of the challenges and opportunities presented by the profession of clinical psychology to take a leadership role in different contexts. (GMC, E, OSIL, R)
