II. Sport Physicals: Students must have a completed MHSAA Medical Historyform and emergency (gold) card prior to try-outs(available in the athletic office in the counseling office). Bring the completed medical history form and gold card to South Athletics for approval before tryouts. The medical history form has a portion to be filled out by a physician at a sport physical on or after April 15, 2016to be current for the school year. Please make sure to have the physician sign and date the card. The medical historyform will remain on file in the athletic office and the gold card will be stamped and returned to the athlete to be given to the coach the first day of tryouts. If more than one sport is played during the year the athlete needs to request the gold card back from the coach at the end of the season for the next season.

IV. Participation Fee: – There is a $120student athlete fee per sport. This fee helps to cover the costs of transportation, supplies, tournament fees and a number of other expenses that are no longer covered in the school district budget. A check issued to “GPS Athletics” should be given to the head coach by the end of the first week of practice after try-outsorthe fee can be paid online through the athletic website (print the email receipt to give coach). A student athlete will not be eligible to compete in athletic contests until the fee is received.
Financial assistance is available to those with significant financial need. Please call the Athletic Office withconcerns.
V. Booster Club and Season Passes for students: Athletic passes for students can be purchased in the athletic office for $60 which are valid for all home sporting events (not playoff games). Parents are encouraged to join the Coaches Club where they will receive a family pass for all events and other perks for $200 (
