Application Procedures & Additional Required Items:

To apply, please submit nine sets of the following items. Please do not bind the sets.

1. Completed Grant Application form, signed and dated by the Chairman of the Board of Directors, Agency Chief, or the Executive Director of the organization.
2. Detailed proposal of not more than two pages which includes the following information:
3. A short introductory paragraph with a concise statement of the purpose of the request and amount of request;
4. A detailed project description covering the issue being addressed, what will be different and why it is important; the outcomes to be achieved; the plans for accomplishing the outcomes and project timetable; capacity of your organization to carryout the plans; if this is a collaborative effort, the role of each partner; number and type of animals being served; evidence of post surgical care plan and procedure; how you will evaluate the success and effectiveness of the program;
5. A brief description of the history, mission, and activities of your organization;
6. If applying for funds for sterilization, a brief description of current sterilization resources in your community. Please include access to veterinary care; average costs of sterilization surgery; availability and cost of low or no cost sterilization programs; types and number of spay/neuter programs of other nonprofit/governmental agencies. If applying for funds for education, please include a brief description of availability of similar education programs in your community.

9. List the award amount, sourceand purpose of any grants you have received from any grant-making body in the last three years.

11. Nonprofit organizations submit most recent IRS 990 and a Copy of IRS 501c3 Determination letter or proof of nonprofit status. This step is not required for government agencies.

2007Grant Guidelines

Mission statement: The Ohio Pet Fund is a 501(c)(3) charitable corporation that supports educational programs concerning the proper veterinary care of dogs and cats, and programs for the sterilization of dogs and cats.

Funding Policies

The following organizations will be eligible to apply to the Ohio Pet Fund for grants:

(a)Animal shelters as defined in section 4729.01 of the Ohio Revised Code, including a facility operated by a humane society or any society organized under Chapter 1717 of the Revised Code or a dog pound operated pursuant to Chapter 955 of the Revised Code, or

(b)Ohio nonprofit veterinary associations that operate programs for the sterilization of dogs & cats, or

(c)Charitable organizations qualified under Section 501(c)(3) of the Internal Revenue Code, the primary

purpose of which is to support programs for the sterilization of dogs and cats and educational programs concerning the proper veterinary care of dogs and cats. To qualify for the grant cycle beginning with an application in 2007, the 501(c)(3) organization’s Articles of Incorporation must include sterilization and education in its purpose statement unless it otherwise qualifies in (a) or (b).

Grants are awarded for one year only. An organization may apply only once per year. In the past grant cycle, all grant awards were under $10,000.

Owners of pets that benefit from grant funds awarded to the above organizations must have one of the following pieces of documentary evidence in their name or in the name of someone in their household, or otherwise meet the guidelines in Section 955.201 of the Revised Code:

(a)Tax return showing owner’s family income does not exceed 150% of the federal poverty guideline

(b)Section 8 housing voucher

(c)Schedule letter from Ohio Works First

(d)Ohio Medicaid card

(e)Letter from Veteran Benefits Administration acknowledging service-related disability

(f)Ohio Direction Card (for food stamps)

(g)WIC card (Women Infants and Children nutrition program)

(h)Benefit letter documenting Supplemental Security Income

(i)Benefit letter documenting assistance from Social Security Disability

(j)Certificate of adoption showing that the animal was adopted from a licensed animal shelter or pound.

(k) Certificate of adoption showing that the animal was adopted from a nonprofit corporation.

Review Process

Applications must be postmarked after July 1 and prior to September 1 of each year. Upon receipt of the completed proposal, the Ohio Pet Fund may request additional information or wish to schedule a site visit. Applicants will be notified of acceptance or denial of their proposal on or before December 31. The award of a grant does not constitute an endorsement of an organization by the Ohio Pet Fund. Approved applicants will receive funding for their projects on or before April 1. Checks will not be mailed until the Grant Agreements are received by Ohio Pet Fund. Grant agreement letters must be returned to the Ohio Pet Fund prior to March 15 or funding will be rescinded unless the Ohio Pet Fund agrees to an extension. Funds not used for the accepted proposal within twelve (12) months of receipt of the funds by the organization shall be returned unless the Ohio Pet Fund, in its sole discretion, authorizes an extension of the time period for use of such funds. Organizations receiving grants will be required to complete an evaluative report within one month of the completion of the funded project/program. The Ohio Pet Fund will likely receive more worthwhile grant requests than it has the ability to fund. A denial is not necessarily a reflection of the quality of the grant request or applicant organization. Applicants who did not receive funding are welcome to submit a grant request the following year.
