SUMMARY OF RISK:
Review the risk measured, and the controls, then please select the relevant risk summary statement:
A / The assessment reveals that the potential risk to health from the use of the plant is not currently significant
B / The assessment reveals that the potential risk to health from the use of the procedure is significant, however controls are in place that reduce risk to acceptable levels / X
C / The assessment reveals that the potential risk to health from the use of the plant is significant. Interim controls are in place to reduce risk to acceptable levels.

Definition of Plant:
Any machinery, equipment, appliance, implement or tool and any component, fitting or accessory used in, or in conjunction with them (SafeWork SA).
Assessment Process:
Complete the information overleaf to:
1. Identify the HAZARDS (see Identified Hazards section of risk assessment table)2. Assess the RISK – the PROBABILITY (likelihood) of an occurrence / 3. And the CONSEQUENCE of the occurrence
Very Likely / Will probably occur immediately or within a short period of time / Fatality / May cause death or loss of facility
Likely / Will probably occur in time / Major / Severe injury or illness or major property damage
Possible / Could happen occasionally / Minor / Injury or illness requiring days off work or minor property damage
Unlikely / Could eventually happen (rare) / First Aid / First aid level treatment
Highly unlikely / Has potential to occur, but probably never will / Negligible / No medical treatment
4. Calculate the overall RISK RATING using the risk matrix
5. Identify the RISK CONTROLS (refer Risk Control Hierarchy) that are already in place or are required to reduce the risk to an acceptable level
6. Review – is the process working effectively to identify hazards and control risk?
Risk Matrix: / CONSEQUENCE / PROBABILITY / Risk Control Hierarchy
Very likely / Likely / Possible / Unlikely / Highly unlikely / Most preferred / Elimination / Is it necessary?
Fatality / Extreme / High / High / High / Medium / Substitution / Is there a less hazardous alternative?
Major injury / High / High / High / Medium / Medium / Isolation / Eg Can the plant be isolated or automated
Minor injury / High / Medium / Medium / Medium / Medium / Engineering / Eg Trolleys to move loads, guards on machinery, fume extraction
First aid / Medium / Medium / Medium / Low / Low / Administration / Eg: Training, Safe Operating/Work Procedure, signage
Negligible / Medium / Medium / Low / Low / Low / Least preferred / PPE-Personal Protective Equipment / Eg: Gloves, respirator, safety glasses, mask
