Please complete the form and hand in at the Christian Service Info session on either April 9 (2:50pm) or 10 (7:45am) 2014 in W200.

minimum of 2 hours each week.
* I will have begun volunteering at my site before my first Christian Service class meets in September 2014. I will volunteer each week through the last cycle of classes in 2015. I understand that if my site is closed, I need to make up the hours for that service session. I understand that the 2 hours I serve in the community is EQUIVALENT to being in class.
* I understand that as per the Parent Letter, that if this contract is notcompleted/signed and turned in on time I will lose5 points off my 1st Quarter final grade.
* I understand that if I have not begun my service by the first class meeting, that I will lose 5 points off my first homework assignment AND first service log sheet. I realize the importance of making this commitment a priority over all other extracurricular activities and I will make all the necessary arrangements to ensure that the people I serve receive all of my attention and energy. I will also offer support to my classmates through my participation in the classroom.
