Permission to Photograph or Videotape

___I give Capital Area Intermediate Unit my permission to photograph, videotape, or digitally record my child, ______for the following

uses:

Still photographs for slide presentations, newsletters, and displays; or motion pictures, videotape, or digital recordings (including voice/and or video) of activities related to the CAIU and used within the CAIU for educational purposes (for example, teacher inservice, evaluation, parent programs) and/or used outside of the CAIU for teacher-training by colleges, other intermediate units, school districts, PTA or other educational, service or support agencies; AND

Still photographs, motion pictures, videotapes, or digital recordings as described above of your child can be used on a Social Media sites such as Twitter, Facebook, and Vine, as well as Web 2.0 sites, such as wikis and blogs. Please note: some Social Media and Web 2.0 sites used are protected and only the parents and teachers will have access to photos, videos, and information pertaining to your child. Some sites will require parents to create an account for access and anyone with an account will be able to view/access the photos, pictures, videos, and/or recordings. In such cases, the child’s name will NOT be used.

*According to the Capital Area Intermediate Unit Social Media Policy 815.2 wikis and blogs are an acceptable form of social media to be utilized by Capital Area Intermediate Unit employees. Employees are expected to follow the rules and guidance set forth in the Capital Area Intermediate Unit Social Media Policy 815.2.

___I do not give the Capital Area Intermediate Unit my permission to photograph, videotape, or digitally record my child.

Any photograph, videotape, or digital recording to be used in a newspaper or on a TV station MUST have prior parent or guardian permission on a separate “Media Release Form.”

Permission to Take Local Walks

During the school year, our class will be taking walks in the vicinity of the ______school building. These walks are used to enhance a variety of educational lessons.

___I give permission for my son/daughter to take walks within the vicinity of the school

building with his/her class and staff.

___I do not give permission for my son/daughter to take walks within the vicinity of the

school building with his/her class and staff.
