It will also provide strategic support to the operational group as required.
That Members note that the role of this group could potentially be to consider the wider development of healthy active lifestyles in Poole, to progressively include sports, exercise and recreation - adopting the concept of a Community Sports Network.

HealthLink was launched in Bournemouth in 2004 for a three-year term funded by Bournemouth Borough Council’s Leisure Services in partnership with Bournemouth Teaching Primary Care Trust and Bournemouth University.
It was formed to help residents attain healthier lifestyles by focusing on three of the most powerful health benefiting activities; physical activity, smoking cessation and weight loss.
HealthLink works closely with a variety of partners from the public, private, community and voluntary sectors, to bring together information on a range of community based opportunities that can enable an individual to take action appropriate to their needs.
HealthLink facilitates two Development Days per year to harness the resources available to maximise the opportunities available for local people.
Local people can access HealthLink by phone, email or web for information and guidance to help them identify an appropriate route for themselves. They can also attend ‘Gateway’ sessions help in health and community settings, which are free, friendly and informal and are provided to help people explore motivation, barriers, goals and actions.
In 2006
Bournemouth HealthLink were winners of the Dorset and Somerset NHS Partnership Award as well as the overall Chairman’s Award in recognition of outstanding work in modernising the National Health Service in Dorset and Somerset and showing exceptional commitment and dedication to the NHS and its patients.
In 2007
Bournemouth and Poole concluded respective physical activity public service agreements.
Bournemouth and Poole Leisure Services recognised the potential benefits in working together on specific physical activity initiatives.
Bournemouth and Poole were successful in securing physical activity local area agreements.
Bournemouth and Poole Primary Care Trusts merged.
Bournemouth and Poole agreed to align their physical activity strategies/action plans.
The Active People survey provided initial participation physical activity data.
HealthLink included as a case study in "Culture Matters" - an advocacy document for Cultural Services in the South West.

24% of Poole adults participate in at least 30 minutes activity three or more days per week.
25% of adults are obese and forecasts indicate this will rise.
A population shift in attitudes and behaviour is needed together with a conurbation wide response to tackle inactivity.
Active HealthLink has the potential to develop and is one way of joining up conurbation-wide initiatives that promote the adoption of healthy lifestyles.

5.2 / The Future: Active Healthlink 2007 – 2011
The objective of the programme is to support the new physical activity Local Area Agreement and the objective of the Poole’s Physical Activity & Recreation Strategy to increase participation levels in physical activity by providing opportunities for local people to create and sustain personal health, fitness and well-being.
To this aim Active Healthlink will:
  • Under new branding as Active Healthlink 3x30 focus predominantly on the promotion of a physically active life by developing and establishing an Active 3x30 club membership scheme for local people. This will provide baseline and achievement data to inform the physical activity local area agreement targets.
  • Also play a supporting role in the following lifestyle actions: weight management; smoking cessation; behavioural change.
  • Establish the Active Healthlink brand locally as the focal point for lifestyle change meaning patients with existing health problems, who are not currently physically active, can be referred to a local Leisure/Activity Centre to learn the skills for exercising effectively.
  • Develop a resource bank of opportunities by forming local Network groups in the following categories:
  • An Active Life
  • A Slimmer Life
  • A Smoke Free Life
  • Change Your Life
  • (We will also form a network group for referral sources
and influencers).
  • Develop and establish an interactive website to support the project. activehealthlink.co.uk
  • As well as providing general access opportunities, Active Healthlink will also focus effort on establishing 3x30 club membership schemes in the following areas:
  • Health and Well Being need
  • Identified Groups and Communities
  • Workplaces
  • Education

Governance – Reporting Lines


Active Healthlink Operational Group:
Fortnightly meeting by core team to review service delivery and ensure agreed milestones are met.

Quarterly meeting to critically assess the project delivery as agreed, its adherence to the agreed milestones and potential to achieve agreed outcomes. It will also provide strategic support to the operational group as required. Members to be confirmed.

Six monthly updates to critically assess the project delivery as agreed, its adherence to the agreed milestones and potential to achieve agreed outcomes and its contribution to the respective sustainable community strategies and the Local Area Agreement. It will also provide strategic support to the Active Healthlink Strategic Group and the operational group as required.

Bournemouth and Poole will be working together collaboratively to develop a conurbation Active Healthlink provision. This has evolved from Bournemouth and Poole securing comparable physical activity Local Area Agreements and recognising the benefits to be gained by the organisations and the community in working together.
Bournemouth and Poole will work together to establish services and deliver on our individual Local Area Agreement targets, whilst retaining individual authority funding and staffing management.
The complexities of establishing and facilitating funding management systems, staff management structures, service level agreements and expected outputs considering the diversity of investment from proposed partners could easily suffocate the intention, development and delivery of the project and its desired outcomes.
The Leisure Services Units for Bournemouth and Poole will lead on the initiative, supported by Bournemouth and Poole Primary Care Trust, Bournemouth University and Active Dorset.
The level of activity in each Authority will be dependent upon the investment made by each Council and supporting partners.
However, whilst the respective initiatives will be managed individually, they will be developed and promoted collaboratively to ensure that Active Healthlink is perceived as a seamless conurbation wide brand/service connecting local people to opportunities to create and sustain health, fitness and wellbeing through a physically active life.
Any future benefits of pooling funding and staff will be assessed at the annual project review.

Poole Proposed Timetable

October – March 2008 Pilot phase of the Active Health Link Scheme.
October 2007
A joint action plan has been produced and is being delivered between Bournemouth and Poole. Weekly meetings are taking place to ensure delivery.
31st October Active Healthlink will be launched internally during a PCT meeting to local GPs. During this pilot phase access to Active Healthlink will only be available to those who have completed their GP Referral prescribed course.
December 07 to January 08
The Active Healthlink Scheme will be expanded and may involve a recruitment drive to those wider than GP Referral.
March 2008
Completion of the pilot phase of Active Healthlink. All administrative procedures, publicity etc will be finalised.
April 2008
Active Healthlink Public Launch event at Upton House.

11.1 / The development of the Active Healthlink will help deliver on the Council’s Corporate objective of Promoting Health & Wellbeing by providing a wider choice and more access to leisure and sports activities, enabling people to a healthy and active lifestyle.
