How toEnroll in Benefits During Open Enrollment

Open Enrollment Begins October 28and endsNovember 20, 2013

To Log In: Log directly onto the ADP Employee Self Service Portal at and go to Benefits > Health & Welfare. OR If you need assistance logging onto the Portal, go to the CCSNH Benefits web site at click on ‘Health and Welfare Benefit System - Online Benefit Enrollment System’ and follow the step by step instructions.

The Home Page
Here you’ll be able to view important information such us your current elections andthe document library.
To begin your enrollment process click on Enroll Today! /
The Summary Page (Review / Change Benefits)
On this page, you can access your current elections and will be able to make any changes to your benefit elections. Simply click on the Benefit area you would like to make a change to.
Please note: If you want to ADD a dependent, please click on Manage Dependents andadd yourdependent.

Medical Plans
(Blue Choice, National PPO and Medical Buy Out)
Once you’ve clicked on the Medical benefit area, you can enroll in either the HRA BlueChoice New England Plan OR the HRA National PPO plan.
Click Done
OR, you may apply for the Buy-Out plan. (Simply click on the “Waive Coverage” on the Medical page and select the Medical Buy-Out Benefit option).
Click Done.
Please Note: When selecting Coverage, your eligible dependents(s) will appear in the Coverage Information section below your election. Checked boxes indicate that you are requesting they will be covered under this plan choice. If you no longer want to cover a dependent, simply uncheck the box beside the dependents name. /
Pharmacy and HRA
Both are considered “bundled benefits” and will automatically be elected with a medical election. There is no need to do anything /
Dental Plan
Here you can enroll in or waive coverage.
Click Done. /
Enroll in Additional Benefits
The following pages allow you to enroll in additional CCSNH benefits:
  • Voluntary Vision.
  • Optional/Voluntary Life for Employee, Spouse and Child.
  • FSA Plans for Dependent Care and Medical care expenses.
Note: Basic Life and AD&D and Long Term Disability programs are benefits provided by CCSNH, you cannot make any changes to these plans.
ClickDone button at the bottom of each page when you have made your elections. /
2014 FSA Plan Enrollment
Reminder: You must re-enroll for 2014; your 2013 elections will NOT carry forward
FSA – 2014 Medical Reimbursement Plan
On this page, enroll in the 2014 Medical FSA Plan.
  1. Select the Plan
  2. Enter the Amount of your 2014 annual enrollment.
  3. Press Calculate Cost.
Click Done. /
FSA – 2014 Dependent Care Reimbursement Plan
On this page, enroll in the 2014 Dependent Care FSA Plan.
  1. Select the Plan
  2. Enter the Amount of your 2014 annual enrollment
  3. Press Calculate Cost.
Click Done. /
Flex Administrative Fee
All participants, in either the Flex Medical and/or the Flex Dependent Care Accounts, will be responsible for a monthly administrative fee of 3.50 per month. The fee will automatically be applied to your election; there is no need for you to do anything. /

Be sure to Confirm Elections once you have made your benefit choices. /

Last Step!
Once you press the above Submit, you will be brought to a Confirmation page. PLEASE MAKE SURE YOU GET A GREEN CONFIRMATION BAR WITH A CONFIRMATION NUMBER before you click Done at the bottom of the page. /
