The Transition Services

Monthly Activity Log Reports

Who should complete the transition services activity log? / The activity log should be completed by the AE program’s transition specialist.
If an AE program has more than one transition specialist, the specialist should consolidate all responses to the activity log and submit one completed form.
What is the monthly due date for submitting the log? / Activity log reports are due each month on the 10th day of the month. The reports will reflect the previous months transition activities.
Where are the transition activity logs to be submitted? / The activity log should be submitted through the address.
Should the activity log be completed in its entirety? / Yes, all sections of the activity log should be completed.
If your transition program does not provide services noted in any of the sections it should be documented by inserting “NA” in the section referenced along with a brief explanation as to why this section was not completed.
Specialist should make sure to include hours used for administrative and planning versus that used for direct student interaction.
Should the activity log be signed? / Yes, the activity log must be signed by the program administrator and the transition specialist for submission.
SECTION I – ORIENTATION(S)
What would orientations include that are provided by the transition specialist? / Orientations for this purpose can include one or more of the following:
  • General program intake orientation in which the transition specialist provides an overview of the resources offered through transition services (postsecondary education preparation and workforce preparation) in addition to the specialist office hours, contact information, calendar of events, etc.
  • Individual site orientations to provide an overview of the resources offered through transition services (postsecondary education preparation and workforce preparation) in addition to the specialist office hours, contact information, calendar of events, etc.

SECTION II – CLASSROOM VISIT(S)
What would classroom visits include that are provided by the transition specialist? / Classroom visits for this purpose can include one or more of the following:
  • Coordination with instructor to provide information to all students about postsecondary education preparation and/or workforce preparation.
  • Coordination with instructor to host guest speaker to provide information to all students about postsecondary education preparation and/or workforce preparation.

SECTION III – FACILITATED SMALL GROUP(S)
What would facilitated small groups include that are provided by the transition specialist? / Facilitated small groups for this purpose can include the following:
  • Sessions with 10 or less students to provide targeted information about postsecondary education preparation, workforce preparation, or community referrals.

SECTION IV – INDIVIDUAL COUNSELING/COACHING SESSION(S)
What would individual counseling/coaching sessions include that are provided by the transition specialist? / Individual counseling/coaching for this purpose can include the following:
  • Face to face sessions with one student to provide targeted yet confidential information about postsecondary education preparation, workforce preparation, or community referrals.

SECTIONV – REFERRALS
Why is it important to track referrals? / It’s important that local AE programs established collaborative relationships and make referrals to the correct professionals for students to get the best possible outcomes.
It’s important to identify the need for additional support as early as possible.
Referrals made will range based on need(s) to include:
  • Community services
  • Testing and assessment
  • Postsecondary support
  • Workforce/labor support

SECTION VI – STUDENT SUCCESS & FOLLOW-UP
Why is it important to track data for student success? / It’s important that local AE programs monitor their data for accuracy. Tracking student success is a coordinated effort between the instructors and the transition specialist.
