JAY HENDRICKSON MEMORIAL SCHOLARSHIP APPLICATION

The completed application form, essay, letters of recommendation and your official transcript must be submitted to your counseling office no later than Tuesday, March 22nd, 2016.

Purpose:

Jay Hendrickson was to be a 7th grade Harwood Junior High student when he passed away from bone cancer in September of 2002 at the age of 12. Jay attended Spring Garden Elementary and was an avid basketball player. This scholarship honors Jay’s positive attitude, his special friendship with fellow students and teammates, and his enduring spirit in the face of adversity. One Jay Hendrickson Memorial Scholarship in the amount of $500 will be awarded for 2016.

Scholarship Criteria:

·  Must have attended Harwood Junior High School during the 8th and 9th grades and either Trinity High School or L. D. Bell High School for Sophomore, Junior and Senior Years.

·  Must graduate from Trinity High School or L. D. Bell High School.

·  Shall have demonstrated positive character traits and good citizenship through school, community, athletic, religious, and/or other related activities or organizations.

·  Must have maintained an academic standard in line with college/university acceptance. The recipients may attend any trade school, junior college, college or university of her/his choice.

·  Financial need will be considered a factor, but will constitute only one consideration. Extracurricular activities will also be considered, but will constitute only one consideration, as well. The dollar amount of the award will be paid directly to the college or Institute of higher learning (recipient’s choice) at the time of registration or to the recipient upon proof of payment signed by the college or institution.

·  Must provide a minimum of two (2) letters of recommendation from a teacher, administrator, employer or religious/civic/community leader.

·  The scholarship must be secured within one calendar year from the time of high school graduation.

·  The recipients, upon advance notification, are requested to be in attendance at the designated meeting of presentation to help honor Jay’s memory.

·  The spirit of this award is not consistent with a recipient who has been awarded a full scholarship. Disclosure of such an award is required.
