DISTRICT AWARDS INSTRUCTIONS
You may submit a maximum of two (2)projects in each of the individual award categories.Use the Civitan International forms for both projects.
Please note that a project can be used in just one area:New, Community Service, Service to People with Disabilities, Youth, Civitan Awareness, Fund Raising under $5,000.00 or Fund Raising over $5,000.00.
District has awards for New Projects, so put “New Project” on any new individual award form and fund raising projects, to have them judged as a new project.
District will give awards for Total projects in each area, so list all the projects in each category and on a second page give a short description of each of them.
District Awards to be Given at the

- District Honor Key (2 for 2015-2016 year)
- Governor Honor Key – one and plaque (2015-2016)
- District Civitan of the Year – plaque (2015-2016)
- District Secretary – plaque (2015-2016)
- District Treasurer – plaque (2015-2016)
- Outstanding Area Director – (2015-2016) – plaque and patch
You will need to send your HOTW awards packet to:

OR you may email them in a PDF Format to me .
NO EXCEPTIONS.
Keep a copy for the club.
I will make copies for the Awards committee.
Club President of the Year
This award shall be bestowed by the Awards Committee on the President of a Club in Good Standing who best exemplifies the three major purposes of Civitan: Service, Knowledge and Fellowship. Each club is encouraged to submit letters to the District Governor to be received by Dec. 1. The award shall consist of a plaque for the recipient and a banner patch for the club.
Applications must be typewritten or computer printed and a maximum of three (3) pages. Criteria shall be:
1. Service contributions of the nominee50 points
A. Leadership to the club.
B. Participation in at least 80% of service projects of the club.
C. Participation in at least 80% of the fund-raising activities of the club.
D. Attendance at the club meetings.
E. Recruited one or more new Civitans into the club.
2. Knowledge of Civitan, with an emphasis on areas in which the nominee is knowledgeable in the ways of Civitan and how the nominee imparted and used that knowledge to the benefits of Civitan. 20 points
3. Fellowship, with emphasis on fellowship activities in Civitan.30 points
A. Participation in social functions and other activities of the club.
B. Participation in District and International functions, such as Area Meetings, Conventions and committees.
C. Other activities of the nominee exemplifying fellowship within the club and outside the club, including visiting other clubs and other activities of the nominee.
