Service Project Evaluation & Tracking Form Instructions

Greetings Sorors,

With the growing need to have better reporting and capturing of our service efforts, all chapters are encouraged to report their service hours monthly using the Service Project Evaluation & Tracking Form (SPET Form) as well as on the annual report. Below, you will find basic instructions on how best to fill out the SPET Form using this method of tracking and reporting.

Service Project Evaluation & Tracking Form

The following are the sections you will need to complete on this form:

  1. Chapter Name, Region, ChapterCity & State, & Reporter
  2. To select your region, simply click on the text box to activate the drop down box. Then, select your Region.
  3. Please be sure to include the Chapter’s City and State in the Location box.
  4. Event Name, Description, and Location
  5. Click on the text box below this title to choose the event name or select other to enter the event name.
  6. Please remember to give us a brief synopsis of the event.
  7. Your full report should be attached and emailed with this form.
  8. Number of Attendees
  9. Please include the age ranges and gender as well in this section for your attendees.
  10. Also, please note how many sorors, philos, and rhoers were in attendance.
  11. These numerical fields will automatically populate for you, so you will not need to total your own figures.
  12. Number of Service Hours
  13. Simply enter the hours needed to complete the project and the total service hours will automatically populate for you based on the number of sorors recorded previously.
  14. Partnerships
  15. Enter any partner organizations, number of participants, and their contributions.
  16. National Program Involvement
  17. Click to check any programs incorporated.
  18. Donations
  19. Click the text box to select yes or no in the drop down box.
  20. To select the donation type payment, simply click on the text box to activate the drop down box. Then, select your donation type.
  21. In the comment section, you can feel free to include the sponsor(s) of that particular donation.
  22. Panelists, Speakers, Facilitators
  23. List any panelists, speakers, or facilitators.
  24. Additional Information
  25. Answer these questions to give us additional information about the success of your program or service project.
