Your request has been considered under the provisions of the Freedom of Information Act. For ease of reference, I will address each of your questions in turn.

1)How do Croydon Adult Services ensure that service users have access to Reablement Services?

We have regular reviews of usage and we have just completed an external review.

2)Please explain how Croydon Adult Services have adopted the Care Act 2014.

Following a programme of design and development Croydon has fully implemented all duties of the Care Act.

3)Please provide me with a copy of the policies and procedures that social workers have to follow when assessing eligibility for Reablement Services.

We are in the process of drawing this up.

4)Do Croydon Adult Services offer this Reablement Services to people who have been recently discharged from hospital?

Yes.

5)At what point do Croydon Adult Services make the service user aware of Reablement Services?

As soon as possible

6)How do Croydon Adult Services record information about service

users who are being assessed or applying to be assessed for

Reablement Services.

We use our client database to record all information on service users.

7)How many service users have been assessed for Reablement

Services?

For reporting period 2014/15 (1 April 2014 – 31 March 2015): The Council is required along with other local authorities to complete a new annual data collection –SALT (Short and Long Term Services), the information that you have requested has been published in the public domain during October 2015 and is available at the following website:

8)How many service users have applied to be assessed for Reablement Services?

We do not record this information

9)Do Croydon Adult Services charge for Reablement Services?

No.

10)Please provide a breakdown of the number of people that have

received Reablement Services for each ethic group.

No, but it is part of their role to do this

15)How do Croydon Adult Services monitor Service Users who have

"protected characteristics" under the Equality Act 2010 when

applying Fair Access to Care policies?
