Jenks High School Screenwriting Class Application for school year 2018-’19

Complete this application by Wednesday, February 28th. You may e-mail it back directly to . You may also turn in a physical, printed-out copy to Mr. Raphael in High School Building 5, Room 5231 (or give the physical copy to your school counselor to return to me). If you come by my room to turn it in and the classroom door is closed or locked, there is a magnetic clip on the door. You may just attach your application to that.

Please note: All students accepted to this class will on occasion be required to read their work—and that of their fellow students—out loud. It is important that you be comfortable with reading aloud in front of other students.

Your current English teacher, including first name if you know it ______

1. What overall grade did you receive in your English class this past semester?______

If you received less than an A or B, please explain why.

2. List two or three of your favorite movies, plus a brief description of why you like each film.

3. Do you ever read books for pleasure, other than required reading for school?

4. If you answered "yes" to Question #3, name two or three books you've read for pleasure that you've really liked, and tell me a few things you liked about them.

5. Please attach—preferably by stapling—a typed page of some of your best creative writing. If you are submitting this application electronically, then electronically attach a Word or PDF version of your writing sample. (You may also copy-and-paste from an existing Word document into this one.) Before clicking “send,” double-check to make sure you electronically attached both your filled-out application and the creative writing sample.

Please provide one page, double-spaced, approximately 200 to 250 words in length. This creative writing sample may be from a short story you’ve written, or from a personal essay. Please do not include an excerpt from a research paper, such as a paper you would write for a social studies class.

Your creative writing sample does not need to be longer than one page. You will not be penalized for providing more than one page—however, I may not read more than your first page, so please make sure that the first page is some of your best creative writing. Instead of an excerpt from a short story or personal essay, you may include poetry or a script (either for stage or screen); however, if you include a poem or script, make sure it is a stronger example of your creative writing than anything else you’ve written. My preference is for a sample from a short story or creative personal essay.

Be sure to have a parent or guardian read and sign below. If you are sending electronically, please ask your parent or guardian to e-mail me from his or her work or personal e-mail address, simply stating that you have permission to watch G, PG or PG-13 films in class.
