WEST SUBURBAN OFFIIALS ASSOCIATION
INDEPENDENT CONTRACTOR AGREEMENT
Nothing in this Agreement shall be interpreted as a guarantee of assignments. This agreement shall govern only in the event West Suburban Officials Association (WSOA) assigns (print name) ______to games scheduled by WSOA. The representations and acknowledgement contained in this Agreement are a prerequisite to working games assigned by WSOA. This Agreement incorporates all written directives concerning protocol and procedures in relation to assignments accepted by the official, receipt of which is acknowledged by

WSOA may, from time to time, assign THE ABOVE OFFICIAL to supply non-exclusive on-ice officiating services for games scheduled and conducted by various area amateur hockey leagues. In order to obtain assignments, THE ABOVE OFFICIAL makes the following representations:
1. That THE ABOVE Official is gainfully employed outside the sport of ice hockey, or if not employed, that THE ABOVE Official does not consider hockey as his/her primary means of employment.
2. That THE ABOVE Official receives all on-ice training from USA Hockey. Training provided by USA HOCKEY is strictly in accordance with USA Hockey Manuals and Procedures
3. That THE ABOVE Official receives on-ice officiating assignments from persons or entities other than WSOA or that THE ABOVE Official is able to make his/her on-ice officiating services available to other persons or entities.
4. That THE ABOVE Official purchases or obtains all necessary on-ice officiating equipment from sources other than WSOA.
5. That THE ABOVE Official has procured major medical insurance coverage or relies exclusively on the insurance provided by USA Hockey, Inc. and will not look to WSOA for compensation as a result of injuries sustained while working games which WSOA assigned.
THE ABOVE Official further acknowledges:
1. That THE ABOVE Official is not an employee of WSOA and that THE ABOVE Official is responsible for all applicable income and self-employment taxes.
2. That WSOA assigns officiating assignments as an accommodation to various area leagues and receives an administrative fee for this service. WSOA does not establish game times, game rules, officiating protocols or other aspects concerning the conduct of the games to which it assigns officials.
3. That evaluations are conducted by WSOA. Assignments are made pursuant to these evaluations.
4. That WSOA is neither employed by nor acts as agent for any area league or USA Hockey.
5. This Agreement does not constitute an employment contract or a guarantee of pay or assignments.
6. All protocols for assignments and pay are set forth in West Suburban Officials Association bylaws, which can be obtained on the WSOA website or the association secretary.
7. That THE ABOVE Official has obtained and read WSOA by-laws and operating procedures. A copy of the bylaws may be obtained from the association secretary or on line at the association website.
