SERVICE-LEARNING REQUIREMENTS
The following requirements MUST be completed and turned into the Student Services Office (345 Student Services Building) or bring them to one of the General Hospital Orientations by Friday, September 13 @ 5pm.

· MSU – register at www.servicelearning.msu.edu, click “find a service opportunity” (print confirmation page for your records)
· Non-MSU – Fill out request form on Website / Must Register EVERY Semester
2 / Completed McLaren Volunteer Application / Complete every semester
3 / Provide written proof of a “Negative” TB (Tuberculosis) test / Test must be administered every 12 months
4 / Completed Background Check Form / Complete every 2 semesters
5 / Attend Hospital Orientation / Every 12 months
6 / Attend a Unit Tour/Training / When volunteering in a new unit
7 / A copy of your student ID / Every semester
HOSPITAL ORIENTATION INFORMATION
New Volunteers (never volunteered at McLaren before):
§ The General Hospital Orientation is mandatory (MUST attend one of following times).
o Option 1
§ Date: Friday, September 6
§ Time: Registration 2-2:30ppm, Orientation 2:30-4:00p
§ Location: Dawe Auditorium. Refer to the map of McLaren for directions.
o Option 2
§ Date: Thursday, September 12
§ Time: Registration 4:00-4:30p, Orientation 4:30-6p
§ Location: Dawe Auditorium. Refer to the map of McLaren for directions.
§ Must attend a tour/training for the new unit.
Returning Volunteers to the Same Unit
§ Do not have to attend the general hospital orientation
§ As long as your requirements are completed/up to date, volunteering begins on Saturday, September 21
Returning Volunteers to a DIFFERENT Unit
§ Do not have to attend the general hospital orientation
§ Must attend a tour/training for the new unit, done the first day of volunteering unless otherwise stated
VOLUNTEER COMMITMENTAs a hospital volunteer for the 2011 Summer Semester, you are required to volunteer starting the week of Saturday, September 21 through Friday, December 6 , 2013 and are allotted a maximum of two absences.
