General Discussion:
Maintenance Therapy / How are agencies approaching maintenance therapy: surgical patient waiting for therapy, chronic patient with CHF needing caregiver to understand and demonstrate competence with care. Also, how is HHA used- benefits vs. Barriers. There is a webinar series presented by the HCNAH- information handed out and attached to minutes.

Presented information “The Value of Psychiatric Home Care” This is an overview for clinicians not trained in the behavioral health program.
Also attached is the Geriatric Depression Scale and the “Homebound” documentation cues discussed in the meeting.
Health Facility Licensing Unit / Mike Fleming and Karen.RN surveyor joined us for the meeting. The list of tags(citations) is attached
- Home Care in NH has strong compliance and good interpretation of COPs.
- There were 45 total citations since January 2014- mostly in the care planning section- related to measurable goals. Plan of corrections and citations are located on the website and are publicly available.
- Survey process is changing and will focus on “core” surveys. Fewer things will be reviewed in more detail and if there are no issues then the survey will end, It should be a good survey process for high performers. Will save resources.20-25% of the current items will be reviewed, shorter surveys. The design is to be collaborative, fair and thorough.
- Acceptance of patients for service. Must provide all services ordered by the MD. If a service is unavailable must address with MD and patient and have orders that reflect that the service is not needed. If the order is for PT/OT and agency does not have OT, then a new order must be received or the patient must be referred elsewhere.
- There was much discussion about the need for all diagnoses on the POC must have a measurable goal- Beth will follow-up with Mike and Karen and report back.
