THE ROLE

Under the direction of the Program Coordinator, the Activity Program Planner develops, plans and facilitates the culture and direction of the CBS activity program. The role provides supervision to Activity Assistants, support workers and volunteers working within the program. The Activity Program Planner role establishes a well functioning work environment that promotes social participation and inclusion, personal independence/wellness and life long learning opportunities for the participants.

This position will contribute to and be guided by Rise's Strategic Plan and will work within established guidelines and delegations.

KEY RESULT AREAS

Area 1:
Program Development and Management / Expected Deliverables Include:
  1. Plan, develop and facilitate a program of activities with participants that are responsive to individual needs and goals
  2. Develop, monitor and review individual client plans/goals
  3. Facilitate and assist with transportation of clients to and from the centre based program
  4. Promote wellness, community inclusion, skills building opportunities, valued and meaningful activities, and positive relationships
  5. Ensure cultural appropriateness of all activities
  6. Contribute to the monitoring of the program's budget

Area 2
Program Administration / Expected Deliverables Include:
  1. Maintain documentation as required
  2. Review, record and respond to complaints and compliments
  3. Monitor program compliance with organisational and service policies and procedures
  4. Contribute to monthly reporting
  5. Maintain accurate and detailed records and case notes for each client

Area 3:
Staff Responsibility / Expected Deliverables Include:
  1. Contribute to the development of staff and volunteers
  2. Participate in program inductions with a focus on cultural appropriateness
  3. Monitor, review and manage staff performance within delegation parameters
  4. Provide mentorship to activity assistants ,support workers volunteers and students
  5. Contribute to formal and informal staff recognition arrangements

  1. Foster and promote a collaborative team environment within the workplace
  2. Enact and promote Rise’s values and ethos in all behaviour, relationships and communication
  3. Apply all Occupational Safety and Health requirements and risk assessment in line with legislative and organisational requirements, policies and procedures

SELECTION CRITERIA

This position requires experience working in a community setting and in planning and delivering appropriate activities to vulnerable target groups including those who are frail aged, living with a disability and/or dementia. Prerequisites include:

Qualifications and Experience:

  • Qualification in Community Services Certificate IV level and/or qualification in a related discipline or working towards qualification
  • Experience in delivering an activity program within a community setting with frail aged, and/or those with a disability and their carers

Skills and Knowledge:

  • Ability to work positively within a team environment and able to work with minimal supervision
  • Experience in building positive teams
  • Experience in the delivery of culturally appropriate recreational and social programs responsive to the individual need of participants and their carers
  • Well developed communication and interpersonal skills with the ability to work with clients, families and advocates. Demonstrated ability to engage effectively with Aboriginal and Torres Strait Island people (Aboriginal centre based services only)
  • A demonstrated awareness and sensitivity of issues associated with older Aboriginal people and appropriate behaviour in working with Aboriginal and Torres Islander people. (Aboriginal centre based services only)
  • A high level computer competency including Word, Outlook and data base experience
