INSTRUCTIONS:

AGENCY INFORMATION

Name of Agency - Required

Agency Address - Address where mail is be picked up and delivered

Daytime Contact (Primary / Alternate) - In case n IMS representative needs to contact the Agency.

NOTE: Please list direct lines and respective extension numbers.

Night Time Contact - For IMS Courier to call in case of emergency (broken lock, alarm issue, broken key in lock,

etc.)

ALTERNATE SERVICE OPTIONS

This section is to identify procedures for the couriers to follow in the event he/she was unable to access Agency or packages will not fit in your provided receptacle.

Contact Night Contact For Instructions - Courier will contact Primary or Alternate for guidance.

Leave Mail at Alternate Site - Courier will leave mail at site designated by Agency.

Return Mail to IMS Mailroom - Courier will bring mail back to IMS and contact will be made with Agency on the next business day for alternate delivery arrangements.

Other - Identify other delivery options in the event the courier was unable to access the agency.

SPECIAL INSTRUCTIONS

List detailed instructions on accessing the building and where to pick up/ deliver mail.
