(and from the Manager of the facility which had the Strep A outbreak but was unable to attend the forum) / Natacha & Mona provided background to the CDHB Community Infection Prevention and Control Service; which has been operating for the past 2 and a half years. The service is an advisory and education service that continues to evolve to meet the needs of ARC and other Community health providers. Currently the team consists of Ruth Barratt, Natacha Maher, Alison Carter & Mona Schousboe. Mona explained her role as Clinical Director, including providing liaison between the Hospital and Residential facilities. She touched on a group called the Canterbury MRSA group operating between 2008 and 2012, which had overseen and assisted with control of MRSA outbreaks caused by a strain called USA300 in three LTCFs.
Mona spoke about the recent outbreak of Strep A in a Canterbury facility which resulted in several older people dying. She stressed the importance of hand hygiene when treating older clients with very fragile skin, as well as ensuring staff have “healthy hands”, free from wounds or skin infections.
Following several questions around the cause of the outbreak Mona stressed that “infection prevention is NOT putting measures in place AFTER something has happened – BUT always remembering to be aware and having protocols in place. However, if outbreak occursin spite of having the best preventive measures in place, it is important to act fast to contain the problem.”

A CDHB policy, “Infection Prevention and Control Discharge Requirements for Community Care Facilities”, is being developed to help Improve Communication of IP&C information between the Hospital and ARC facilities. The policy will be available to ARC facilities via the CDHB website once approved.
Addition: key factors from ARC perspective
- Identifying cells within the facility and having staff allocated to an area and not crossing from one cell to another.
- Data base for relatives this made the daily updates to residents’ families far easier as we emailed 85% and therefore had lower numbers to ring.
- Need to have a centralised point to co-ordinate from.
- Paid leave for those needing time off, this ensured staff were open with any small cut or wound.
- We all check our hands as part of routine handover to ensure even a paper cut is covered.
- Team work by all staff, relatives and DHB.
- The support from the DHB and the expert knowledge that was imparted from the team we are eternally grateful it gave us strength in what werevery trying times

Anna talked about SIAPO’s recent analysis of the Medication Management Audit findings from Aged Residential Care Certification and Surveillance Audits in all five South Island District Health Boards.
Currently looking at the most effective response to the information gathered.
While attending meetings she said one of the main concerns raised was issues with GPs and medication charts.
Several members at the forum agreed that it is difficult to deal with a GP who won’t comply.
Anna referred to the Medicines Care Guides for Residential Aged Care (MOH, 2011 Page 14, under the heading of ordering. It states ‘Each medicine ordered must be signed and dated by the prescriber” the interpretation of this is that each medication is individually signed.

- The Health of Older Person’s workstream has appointed a subgroup: members include Kate Gibb (Chair), Rose Laing (Primary Care Liaison GP), P&F rep, GPs, 2 pharmacists, nursing and ARC representatives). This group has met once to discuss the findings of Anna’s audit and also in response to Don Mackie’s national request that DHBs review medication management practices in ARC. Further meetings are planned. / (see attached presentation)
