Before completing the form please note the following:

  1. SCAN Cancer Audit datasets are collected and registered for audit purposes. If you wish to use the data for research it will be necessary to obtain Caldicott Guardian permission for use of the data for this purpose.
  2. The audit datasets are stored within Lothian University Hospitals Division network and are subject to the NHS Lothian eHeatlh Security Policy which prohibits the storing of division data, most particularly patient identifiable data, on any other media than the Division network LAN drives.
  3. In line with the eHealth Mobile data Protection standard all Laptops in use within NHS Lothian are encrypted using a FIPS 140-2 approved full disk encryption product. This software prompts users to enter their log in credentials prior to the system booting.

4.The storing of patient identifiable data on mobile devices (laptops, memory sticks, PDAs etc.) is not permitted without the express permission of the Caldicott Guardian.

  1. Current policy allows for sending patient data on email within the encrypted nhs.net email system. Data should not be sent via or to any other email system or addresses. In particular the EdinburghUniversity email system is an outside system and is not regarded as secure for transmission of patient data.
