Appendix 2: Supplementary questionnaire
A. For all patients,
- Is there a prior history of urinary tract infections?
- How many?
- At what ages?
- What age was he/she toilet trained?
B. For those with late onset (juvenile) Pompe disease and having no current problems,
- Did he/she have any problems prior to diagnosis of Pompe?
- If so, Please elaborate
- Did ERT help symptoms?
C. For others,
- Does laughing/giggling cause urinary leakage?
- If no, next question
- If yes,
- Quantify amount of leakage
- Small spot in undergarment
- Medium amount
- Large amount- consistent with full bladder emptying
- Quantify frequency
