The Fees are stated exclusive of GST (as defined under the A New Tax System (Goods and Services Tax) Act 1999 (Cth)). If GST is payable on any supply made under this agreement, the recipient of the supply must pay to the supplier an additional amount equal to the GST payable.

Unless otherwise stated, the prices quoted in this document are in Australian dollars.

Cancellation Fees

If a representative from your companyor an examinee wishes to cancel an appointment for any reason, you must ensure we receive notice of cancellation in advance of the appointment time, as follows:

Specialist appointments / 2 clear working days
Bulk appointments (3 or more appointments booked at the same clinic for the same day) / 2 clear working days
Scheduled site visits / 2 clear working days
Other appointments / 1 clear working day

You will be charged a cancellation fee equal to 80% of the Fee for the Services scheduled to be provided, if:

the Examinee does not attend his or her appointment in a Medibank Health Solutions Clinic; or
we do not receive the required period of notice of cancellation, as set out above.

In addition to the cancellation fee(s), if you cancel a scheduled site visit for any reason, irrespective of the period of notice, you will also be charged for all other unavoidable costs we may incur in connection with the site visit, such as non-refundable travel and accommodation costs. We will use our reasonable endeavours to mitigate these costs.

A cancellation notice is only deemed to have been received once we confirm to you that we have received it. You may notify us of a cancellation by calling the relevant Medibank Health Solutions Clinic or booking service or by sending an email to the relevant Medibank Health Solutions Clinic (you can find the email address for each of our clinic locations at
