The focus of this practice is to enable the student to develop their learning

in the workplace in relation to University-based modules at Level 3.

This assessment booklet is individual to the practice. It is in the form of an extended learning contract. Students and professional practice educators are expected to negotiate weekly learning outcomes, which build towards achievement of the final learning outcomes, which are stated on pages 12, 13, 14 and 15. The professional practice educator should note learning resources available to the student and the agreed method of demonstrating that a learning outcome has been achieved. Weekly evaluation sheets are intended to be used flexibly to meet individual student’s needs. There is no requirement for a set number of learning outcomes each week.

All learning outcomes must be achieved by the end of the professional practice.

Progress is monitored by completion of the weekly evaluation sheet, which then becomes a formative assessment. In addition to achieving the identified learning outcomes the student must conduct him/herself in a professional manner as set down in the current HPC Standards of Conduct, Performance and Ethics and the COT Code of Ethics and Professional Conduct and abide by organisational health and safety policies. If weekly monitoring indicates that the student is unlikely to meet the outcomes or is failing to meet the required professional standards then the professional practice co-ordinator at university must be contacted immediately.

Student must submit ALL COMPLETED FORMSVIA MOODLE

as per the Hand-In Checklist on page 1.

FAILURE TO DO SO COULD COMPROMISE THE RATIFICATION OF RESULTS

Students should keep copies of the paperwork for their portfolios.

For the duration of the programme of study, this booklet remains the property of York St John University
