2.If a current eighth grade student, the student asks one teacher or his/her principal to complete a recommendation form.

3.Parents file the Student Aid Form with Private School Aid Service.

4.All forms are returned to Cardinal Mooney High School by April 15, 2016.

N.B.To be considered for a scholarship, a student must be registered for

Cardinal Mooney High School.

Directions:For each of the criteria below, write a short explanation which describes how you meet each of the qualifications. Since these are competitive awards, make your description as specific as possible. For example, include club or activities' names, offices held, any honors you may have received, and projects in which you are involved. This information will be important in helping the members of the Selection Committee make their decision.

1.SERIOUSNESS ABOUT ACADEMIC WORK: consistently putting forth effort in your classes and working to develop your God-given abilities.

2. PARTICIPATION IN SCHOOL ACTIVITIES: participation in clubs, projects, athletics, class

activities, and school or community organizations.
