SUBJECT:SUPPLEMENTAL SICK LEAVE BANK – 2012-13 OPEN ENROLLMENT

All eligible employees may participate in the Supplemental Sick Leave Bank (SSLB) program. To participate in the plan, submit the attached Membership Enrollment and Termination Form and have available one day of local personal leave to contribute for the 2012-13 program year (9/1/12 through 8/31/13).Current members enrolled in the SSLB plan and employees enrolled during the pre-enrollment period (June 2012) do not need to reapply.

The SSLB is a voluntary program providing participants an opportunity to request up to 30 additional leave days in the event of a catastrophic illness (defined by the plan) resulting in an absence from work exceeding 7 days during the program year. Participants must exhaust all vacation, compensatory, state, and local personal leave before entitlement to SSLB payments. More information is provided in the linkedQuestion and Answer page.

The qualifying local leave day must be available and will be deducted following the open enrollment period. It is the participant’s responsibility to verify the contributed day has been deducted to ensure membership in the SSLB program. Employees with no available local personal leave on the date the deduction is made in PeopleSoft will be ineligible to participate in the plan year (even if a leave day was available when the Membership Form was submitted).

The district offers three SSLB enrollment periods providing all eligible employees an opportunity to participate, or terminate from the program; the pre-enrollment period was in June 2012. Additional open enrollment periods are conducted in August and November 2012.
