We would like to encourage all 4-H members, parents, advisors, and key leaders to nominate individuals for our county 4-H volunteer awards. If you know someone who has made a positive impact on our 4-H youth and 4-H program as a whole, do not let it go unnoticed! You’re nominations are important! Help us recognize excellence in 4-H!

DIRECTIONS:
- Fill out the nomination information on the back of this form.
- Attach a written summary of why you think this 4-H volunteer deserves the award. Remember that the more information you give us the better. Your summary gives the judges insight into someone they may or may not know!
- You may nominate 4-H volunteers (advisors/resource leaders) for each of the categories, but make sure to attach a written summary for each one.
Tips on what to include in a nomination summary:
- Nominees 4-H accomplishments;
- Specific examples of the impact the nominee has had on the lives of 4-H members;
- Community Service;
- Specific examples of the impact the nominee has had on the 4-H Program as a whole;
- What is different, special or unique about this volunteer which sets him or her apart from other 4-H volunteers.
