Serious Incident – Property Damage

Location: 90.5 km Nadina FSR

Date of Incident: Dec. 3, 2010

Details of Incident: Collision of an Empty Transport truck and a Loaded Log Truck.

-Loaded log truck left block access road, after using stamp hammer, in the down direction and called “on at 92km and down”, as the truck was pulling on the Nadina FSR as a pickup was reaching the intersection at the same time in the down direction. The log truck slowed and let the pickup go a head, the pickup apologized for having to slow the truck down and began to chat about the availability of some logging trucks. After the log truck had traveled approximately 200m down the road another pickup traveling down came behind the log truck and the truck let the pickup by. During this time a transport truck traveling in the up direction (Up truck) was waiting at 89.25km. The Up truck heard the “94km down” call and the “on and down at 92km” call and some chatting. The pickups did not call any more km’s after 94km. The first pickup passed the up truck and did not advise the up truck of any other down traffic, the up truck began to lift off and had to stop when the second pickup appeared. Thinking these were the two voices of 92km call and 94 km call and of the two vehicles chatting, the up truck began to travel in the up direction. Since there was no call for both ways, down for three or an up call, the up truck and the down log truck did not know each other existed. The road section between 91-90km is a curvy and hilly section of road. The two trucks met on a corner at 90.5km. The up truck had applied some brakes when seeing the log truck which caused the B-Train trailers to swing out and into the down lane, which the log truck collided with. The impact of the collision forced the log truck into the ditch and then lost its load. There was extensive damage to the log truck and to the transport truck. Since the down truck was not yet at 90km it did not have the opportunity to make its next call.

- The pickups were advised of the accident and did not return to the scene. They did not have any ID plates on vehicles and could not be contacted.

Recommended Preventative Actions:

1.  All Vehicles must follow the Districts Road User Calling Procedures.

a. Vehicle’s must call in convoy’s when multiple vehicles are within a km of each other

b. Vehicle’s must call both ways when meeting up traffic

c. Up vehicle must call their location every 5km or at must call signs

d. No chatting on road channels

2.  Must call up sign’s should be installed at all major junctions.

3.  Revise calling procedures to include vehicles entering road systems must call the first two down km’s then alternating km’s at a minimum to ensure their position has been clearly announced.

4.  Vehicles involved with causing an accident must return to the scene for the investigation.
