Position Title: Community Connections Facilitator
Location: Madoc, ON, Canada
Duration: full-time, 36.5 flexible hours per week
Rate of Pay: As per the Collective Agreement Pay Grid

Position Summary:
As the Community Connections Facilitator you will be responsible for facilitating opportunities for individuals to buildlife skills, community connections, peer relationships and natural supports through various individual or group activities.
Responsibilities:
- Create and/or cooperate in the development of supports and activities for the Day and the Transition Programs.
- Network and work collaboratively with community members, community organizations and agencies to promote opportunities for individuals.
- Actively seek out and support the development of natural connectionsfor individuals supported.
- Support individuals to build their peer network by promoting opportunities for peer connecting.
- Support individuals to develop decision-making skills, exercise choice, and assume responsibilities and reasonable risks.
- Develop monthly calendar of community recreation, leisure and educational opportunities for agency use.
Qualifications
- DSW diploma or an acceptable combination of relevant Human or Social Services education, training and experience along with 1-2 years’ experience supporting people with developmental disability in a community or recreational setting.
- Current Standard First Aid and CPRand NVCI required.
- Pharmacology certificate.
- Approved Criminal Reference Check.
- Excellent organizational skills and interpersonal skills.
- Excellent verbal and written communication skills.
- A working knowledge of e-mail, internet and word processing and calendar programs.
