IMPORTANT INFORMATION ON CHANGE REQUEST
The fully completed change request form is to be sent to the component responsible MTU buyer.
Change requests are subject to quality assurance standard MTQ 5003 (link below).
If the "MTU Specification Change" checkbox is selected, MTU Purchasing will make a change request (CR) and attach it to the amendment.
The change request must be sent as a Word document (no signature required).
MTU reserves the right to charge processing and documentation costs in accordance with MTQ 5003.

It is not permissible to implement the change. Consultation on further procedure must take place.
- Sample inspection with Cover Sheetwithout component(for information only)-> For Cover Sheet template, use link above
Conditions: - The supplier must ensure that the component specifications continue to be met.
- The supplier must ensure that the change does not infringe the Quality Assurance Agreement (QSV) - also applies to sub-suppliers where applicable.
- The change is to be validated and approved by the supplier and he must compile documentation thereof. The documents will be submitted to MTU when required.
- Information to MTU when the change is implemented in the form of:
Initial sample inspection with MTU Cover Sheet (max. 5 part numbers per Cover Sheet) without PO/component delivery as 1 PDF-document (per Cover Sheet) consisting of:
MTU Cover Sheet (required), agreed change request document (required), further documents (optional)
The supplier shall forward these documents unrequested to the following e-mail address – – with a copy to the responsible purchaser.
- MTU take note of the change by signing the Cover Sheet without component inspection.

Unless otherwise specified, only the change is to be inspected.
- Complete initial sample inspection as per MTQ 5003 -> Cover Sheet + Test Results + drawing with MTU stamp etc. (seelink above)

If several part numbers are affected by the change either a complete initial sample inspection or a sample inspectionfor one example component will be sufficient. These must be coordinated with the MTU Purchasing Department. For the remaining part numbers, the procedure 'Sample inspection with Cover sheet and without component' can be followed once the initial sample has been approved.
