- Position Summary
The Tenancy Support Program works with tenants in the Alice Springs Region, to identify issues that may be affecting their ability to maintain their tenancy.
Through educative strategies; connection to appropriate services and case management and support, the TSP worker will build relationships with residents that support them to navigate their rights and responsibilities as Territory Housing tenants.
- Responsibilities
- Delivery of case management service to identified clients.
- Maintain confidentiality in relation to individual clients.
- In collaboration with clients provide referrals to appropriate services
- Good record keeping and data collection
- Comply with all legislative, funding and organisational policies and procedures
- Work collaboratively with Territory Housing sub-contractors and other service providers.
- Major Accountabilities
ACCOUNTABILITIES / PERFORMANCE INDICATORS
1.Delivery of a case management service to clients.
- Client files are keptup to date.
- Each client has a case plan.
- Clients are included in the case management process ie.development of case plans
- Client files are current and up to date.
- Case Management training provided for TSP staff.
- Case management processes are used.
- Professional development opportunities undertaken.
2.Maintain confidentiality in relation to individual clients.
- Tangentyere Councils policies and procedures relating to client confidentiality are understood and implemented.
- Client information is handled with the sensitivity.
- Client hard copy and e-files are treated confidential documents.
- Client information is stored appropriately.
3.In collaboration with clients provide referrals to appropriate services.
- Advice and assistance in identifying the appropriate agencies provided to clients who need additional services.
- Appropriate referrals offered to clients
- A comprehensive case plan identifies where a client may need additional support from other agencies.
- Referrals by TSP staff made to appropriate agencies.
- Client receive additional services where required.
4.Good record keeping and data collection
- Client files are kept up to date and tidy.
- The SHIP database is used to manage client files and case management plans.
- Case notes are made in a timely manner.
- The SHIP database is understood and used to manage clients.
5.Comply with all legislative, funding and organisational policies and procedures
- Tangentyere Council policies and procedures understood and are used in practise.
- Knowledge of housing legislation and rules and regulations are maintained.
- Relevant Tangentyere Council policies and procedures are followed.
- NTG Territory Housing tenancy rules are understood and followed.
6.Work collaboratively with Zodiac Services, Territory Housing and other service providers.
- Develop a good working relationship with housing sub-contractor.
- Develop a good relationship with Territory Housing.
- Have a good understanding of what services are available within the Alice Springs region and at Tangentyere Council that may be able to support your clients or provide assistance to them.
- Good work relationships are maintained with other services ie. procedures and protocols are followed
- Meetings are attended where applicable
- Appropriate referrals are placed for clients with necessary services.
- Clients are linked in with services as necessary.
