PD Material Pre-approval Request Form

Instructions: Pre-approval requests should be submitted Uat least FOUR WEEKS prior to desired delivery of materials. Priority will be given to complete requests; incomplete requests will be held pending receipt of additional information prior to approval. The more detailed the information submitted with a request, the more quickly we are able to respond. Please Note: Title II funds may not be used to reimburse or purchase materials defined as “curriculum;” that is, any materials which will be used for instruction or placed in the hands of students. Materials that primarily contain student learning ideas/activities are also not allowable. The Title II office of the Minneapolis Public Schools will make the final determination whether a purchase is an appropriate use of Title II funds.
