Biological safety training for lab personnel working with biological agents, toxins or rDNA includes the following actions (as applicable). Please fill in the completion date for each of the required actions and maintain this completed form with your training records for personnel. Please note that all training requirements must be completed within 30 days of personnel beginning work in the laboratory.

Please review the following items with new lab personnel. The purpose of this training is to ensure proper work techniques, safe and effective equipment use, and a workplace where laboratory-acquired infection and contamination concerns are minimized.
Specific Work Practices
Discussion of procedures that involve handling of biological materials (i.e., cultures, tissues, samples) and how to perform such tasks in manner that reduces risk of exposure and environmental release.
Personal Protective Equipment (PPE)
Explanation of what kinds of PPE are required for specific tasks
How to use PPE, where PPE is stored and how to obtain more
Maintenance and storage of PPE
Equipment (Primary Barriers)
Location and operation of safety equipment in lab including eyewash stations, fire extinguishers, gas shut off valves, etc.
Proper use of equipment that is used for the processing, containment and analysis of biological materials including biosafety cabinets, centrifuges, microtomes, cell sorters/counters, automatic sampling devices, incubators, vacuum/suction systems and autoclaves.
Waste Handling
Discussion and clarification of which wastes generated in the work area are biohazardous and how those items are to be segregated, stored, transported, treated and disposed of
Review of procedures for on-site waste treatment (i.e., proper use of autoclave, disinfection of cell culture waste, pipettes, etc.)
Disinfection/Spill Response/Exposure Incident Response
Review of laboratory procedures for disinfection, including explanation on product hazards, preparation, limitations and shelf-life
Review of spill response procedures including location of the biohazard spill kit/supplies and how to use them
Review of Biological Exposure Incident Response procedure
