Dear Sir,
Rex Davis & Associates would like to thank you for the opportunity to furnish your firm the following proposal. The proposal is based on the following architectural drawings and information Rex Davis & Associates has received from your firm: Faxed Drawings from Commercial Wood Products, received 3/6/02, and conversations with Todd VanRoosendaal on 3/7/02.
SERVICES TO RENDER
Included:
- Set of drawings per architectural drawings and conversation with Todd VanRoosendaal to establish scope of drawings. To include drawings of the following: Addition to Kitchen rm#238 and Mailroom rm#232 for The Pointe @ Park Centre.
- One (1) revision if needed after submittal and red lines returned.
- Delivery of drawings

TERMS:
- Total due at time of delivery.
- This Proposal is valid for thirty (30) days.
NOTES:
- Preparation and delivery of the product will take approximately 4 working days +- after receipt of the following:
- Signed proposal.
- Sufficient information to allow completion of project.
2. Rex Davis & Associates require the utilization of precise schedules in order to maintain maximum efficiency. Therefore, all elements of our pricing, including design, material production, and delivery of product are based on the work being performed on the schedule set forth in our original bid documents unless otherwise modified by mutual agreement between Rex Davis & Associates and the Owner and/ or Contractor. Delays in the schedule caused by events not within the control of Rex Davis & Associates, including but not limited to the following, will be subject to price adjustments and will be rescheduled for production and delivery on a best efforts, but not guaranteed schedule, unless otherwise stated in writing.
- Late Receipt of Owner’s architectural drawings or information needed to complete work.
- Rex Davis & Associates shop drawings or submitted materials held for review longer than ten business days after submittal.
- Revisions or changes to the scope of the work prior to, during or following production of part or all the work.
3. Rex Davis & Associates will give Owner or Contractor written notice of price changes within five business days of Rex Davis & Associates receiving knowledge of any events resulting in schedule delays.
- Rex Davis & Associates prices are predicted upon one delivery date for all work pertaining to a project unless otherwise agreed to in writing. Deliveries in addition to those agreed upon in the contract will be billed to the Owner and/ or Contractor, through standard billing procedure.
- All drawings are the property of Rex Davis & Associates. Rex Davis & Associates will supply one (1) set of shop drawings for submittal. If more sets are requested they can be provide at a cost of $1.00 per sheet. Any copies to be made from those drawings or files provided by Rex Davis & Associates, must be approved by Rex Davis & Associates.

NOTES (cont.):
- Rex Davis & Associates must be notified in writing of any proposed back charges or punch list items within five business days of the completion of the project. Failure to give Rex Davis & Associates timely notice will relieve Rex Davis & Associates of any responsibility with respect to such items.
- All orders, including requests for additional work, changes to existing work and associated changes in contract prices must be signed by an approved supervisor for the owner or contractor and by Rex Davis & Associates.
I accept this proposal along with the terms and conditions as stated above.
