This Affiliation Form must be completed IN FULL AND IN BLOCK CAPITALSand returned with the Herts FA Affiliation Form "A" and the Statement of Accounts together with the correct sum due by cheque made out to the “CSSFL”.

All Clubs must affiliate with Herts FA. This is done on the Clubs’ behalf en-bloc by the CSSFL.
Your Club is required to keep a Minutes Book, Rules and Records.
These may be called for at any time by the Herts FA or the CSSFL.
Affiliation Forms will NOT be accepted without a Statement of Accounts.
New Clubs which have not been affiliated to any County FA
may enter "New Club" across the Statement of Accounts form.

A LATE SUBMISSION TO THE HERTS FA WILL NOT BE CONSIDERED FOR INCLUSION INTO THE CSSFL, BUT MAY BE CONSIDERED BY THE HERTS FA FOR INCLUSION INTO THE SENIOR, INTERMEDIATE OR JUNIOR SUNDAY CUPS.
New Clubs: A £25 fee is also due, which will be returned if application is un-successful and returned at the end of the season as a bond if successful. New Clubs Meeting is Monday 10th May at the Shephall Centre.
