Instructions for checking organization information on GetTested.cdc.gov:

1.  Go to GetTested.cdc.gov (http://gettested.cdc.gov) and enter your organization’s ZIP code or city and state.

2.  Look for your organization on the search results page.

If your organization is listed, follow the directions below. If you do not see your organization, skip to Section 2 on page 3.

Section 1 – Review and Revise an Existing Record

1.  Click your organization name to review details.

2.  If changes are needed, click “Update your organization on this site” located in the upper right hand corner.

3.  An online form will appear in a new window, populated with the current information listed in the database. Enter the information requested on the “Before You Get Started” screen and click “Continue” at the bottom of the screen.

4.  The form is divided into five sections with a tab for each. Review the information on each tab and make any changes needed. Then click “Continue” to move to the next tab.

5.  Once you get to the “Confirm” tab, review the information and click “Submit” at the bottom of the page to send your changes to the NPIN Database Team. After clicking “Submit,” a ReCaptcha box will display. Enter the requested phrase and then click the “Continue” button to complete the submission. Your changes will be reviewed by the NPIN team so will not be reflected on the site immediately.

Section 2 – Add an Organization

1.  Click on the “List your organization on this site” link located at the top of the Search Results Page on the right hand side.

2.  An online form will appear in a new window. Enter the information requested on the “Before You Get Started” screen and click “Continue” at the bottom of the screen.

3.  The form is divided into five sections with a tab for each. Enter all requested information on each tab, then click “Continue” to move to the next tab.

4.  Once you get to the “Confirm” tab, review the information and then click the “Submit” button. After clicking “Submit,” a ReCaptcha box will display. Enter the requested phrase and then click the “Continue” button to complete the submission. Your submission will be reviewed by the NPIN team, so changes will not be reflected on the GetTested site immediately.

The forms can also be used to provide information or feedback on an organization other than your own.
