Mustang High School Video Production Contract

I, ______agree to adhere to the following list of expectations as a member of the MHS Video Production team.

To work as a team member and not as an individual player for the entire time I am a member of this team.
To help any team member that is in need of assistance.
To treat all members of the Video production staff in a professional manner.
To complete all Eventsthat I commit to.
To accept any suggestions or constructive criticism that is given by the team or by the advisor with integrity and professionalism.
To show respect for myself and other team members, their work, visitors, and the Video Production advisor.
To behave in a professional and non-confrontational manner at ALL TIMES.
To follow all of the rules and procedures put into place by the video production advisor, as well as all the rule and procedures put into place by the Mustang High School Administration and Mustang Public School District.
To use the computers for editing and school class related work only, no gaming or internet surfing

I agree that if I do not follow the aforementioned professional requirements, that I will accept, without question, the discipline designated by the video production advisor.

Furthermore:

I understand that I will be responsible for covering events that will happen outside of regular school hours.
I understand that I will be using expensive equipment and may have to check out equipment overnight. I understand that I have to earn the right to checkout equipment. I will take care of this equipment and will ask questions if I am unsure how to operate any equipment. I will report any issues no matter how small (i.e. losing a lens cap) to the advisor immediately.
I understand that I represent not only myself when I am on assignment but also the entire Video production staff, teacher, and the reputation of our school.
I understand that I must be eligible in order to cover assignments.

Replacement Costs for Lost, Stolen, or Damaged Equipment

(Be responsible with the equipment and you will be fine)

Canon Video Cameras (Vixia Cameras)------$900

Canon DSLR Kit (Camera, Lens, Battery, tripod, mic)……..$650

Tripods (Basic without fluid head)------$20

Batteries------$10

I ______have read the contract that my student has signed and I am allowing my student to sign it. I agree to all of the elements of this contract and will do my best to support my child while he/she is a member of the MHS Video Production staff.
